Mozang. District Lahore under the name of Sirona Health Inc , do hereby give this warranty that the
drugs described in this invoice do not contravene in any way the provision of the section 23 of Drugs
Act 1976.

ii) FORM-5 (See rules 6(2)(1), 6(5)(b), 19(7) and 48(1)(1)) WARRANTY UNDER MEDICAL DEVICES Rules.
2017, I Sheikh Badar Alam being a person resident in Pakistan carrying on business in aforesaid address
under name of Sirona Health Inc. holding valid license issued by Licensing Authority and having authority
of being authorized by Manufacturers / Principals Vide letters. do hereby give this warranty that the
medical devices here above described as sold by me and contained in the bill of sale, invoice, bill of
leding or other document describing the medical devices referred to herein do not contravene in
anyway the provisions of the DRAP Act. 2012 and rules framed there-under.

iii) WARRANTY UNDER ALTERNATIVE MEDICINES AND HEALTH PRODUCT (enlistment) Rules, 2014.[See
rules 10 (3) & (5) ] We, as the authorized distributor/agents and on behalf of the Principals /
Manufacturers / Importers hereby give warranty that the supplied alternative medicines and health
products mentioned herein do not contravene any provision of the prevailing DRAP Act and rules
framed thereunder.
