1.1. Purpose:
The HSE Plan shall cover all work performed by BMB and its contractors on site. The HSE Plan applies to
all work activities and employees under the control of BMB and its contractors.
The purpose of the HSE Plan is to describe the HSE roles & responsibilities and give detailed guidelines
on HSE procedures that are required to be followed and the proactive actions that are required to be
taken by the project management during the project execution stages. Each BMB employee is
responsible and accountable for HSE activities within the scope of their work.
1.2. Scope:
Bader Al Mulla & Bros Co.S.P.C. has been awarded “Operation And Maintenance Services Work at KNPC
Project contract No. HO/GS/0465.by KNPC. Under the main contract, BMB will be executing Buildings And
Facility Maintenance
This Health, Safety & Environment Plan is applicable to the scope of work mentioned in the Project
titled “Operation And Maintenance Services For MSEOC Buildings And Facility At KNPC”, Contract No.:
HO/GS/0465.
The policy is implemented with commitment from the Top Management at all levels of business activities.
It is issued and periodically reviewed by BMB management and signed by Managing Director. The policy
addresses BMB’s commitment on Quality, Health, Safety and Environment and it gives a direction to
achieve excellence in QHSE. Adequate awareness on BMB’s QHSE Policy is ensured during the BMB
QHSE Induction program.

Objectives:
Each employee that works on or visits the project shall conduct him or herself in a safe and healthy
manner and ensure that the environment is protected. It is imperative that:
Goals:
BMB ensures the leadership to implement the Health, Safety and Environment Plan and regulations at all
levels. Roles and responsibility of all BMB personnel have been defined. The project management i.e.
Managers, Engineers and Supervisors are responsible for implementing HSE systems and procedures at
work sites.
Project Manager is accountable to implement the KNPC and BMB policy and safe work procedures. They
shall consider safety as part of their regular business activities. Job Supervisors are responsible to ensure
safety in their allotted jobs. Safety personnel shall also ensure the adequacy of the safety compliance at
worksite. Roles and Responsibilities of all personnel in the project have been defined and included in the
HSE plan.
3.1. Stopping Unsafe Work:
Employees are authorized to correct the unsafe behavior of company employees and can stop work on
detection of an unsafe condition. Employees are advised to follow safe work practices and are supervised
by competent persons.
Using Form, the observers must fill in the time and date of the observation and the job being observed.
The observers must then clearly describe the job to be observed. While observing the job, notes will be
taken regarding actions or inactions that require correction.
HSE personnel are the part of project implementation team. HSE personnel will advise Project
Management regarding the effective implementation of Health, Safety and Environment issues during
execution of project. Contract HSE organization chart is attached herewith.

HSE roles & responsibilities of site personnel have been defined and enclosed in this document.
BMB’s Site Project Manager has overall responsibility for implementing the HSE plan and it is his
responsibility to ensure that all personnel have the authority and access to the expertise needed to carry
out their HSE responsibilities.
Project Manager:
Leader at site and overall responsible for the implementation of HSE Policy, Plan, Principles, and
Objectives.
Provide overall direction and support for the implementation of the HSE Management System.
Provide sufficient resources to ensure fulfillment of contract HSE requirements.
Review the contract and approve the allocation of resources for practical and effective HSE
implementation and execution.
Ensure the correct functioning of their project site HSE performance and provide performance
evaluation and reports as required.
Review HSE performance and participate in incident reviews and steering meetings as deemed
necessary.
Review the training needs for the project and encourage HSE training for the employees.
HSE Manager:
Advise project management in implementation of HSE policy, plan & procedures at site.
Responsible for providing HSE leadership for the project
Responsible for seeing hazards and unsafe work are indentified and mitigated
Coordinate and facilitate the thorough investigation of all incidents and accidents
Communicate directly with management to facilitate the timely receipt of incident prevention and HSE
information.
Assist Project Management in recruiting, training, and evaluating HSE professionals.
Development or modification of HSE plans during the project as per requirement.
Verify correct functioning and maintenance of the HSE management system through scheduled
review and audit of the HSE programs, procedures and forms.
Responsible to coordinate and comply with the KNPC HSE requirements.
Conduct work place HSE Audits and Inspections as applicable.

Review risk assessments and ensure risk assessment are being followed.
Monthly HSE meeting at site and participate in KNPC HSE meetings.
Organize and conduct HSE training programs to all employees.
Implement occupational health programs and medical examinations for Site Personnel as per
requirement.
Monitor Safety Engineers/Supervisors daily site HSE inspections to ensure that only safe working
methods/procedures are applied.
Ensure actions are taken to close environment inspection /audit findings and submit report on time to
KNPC.
Review & submit Safety Statistic Report, Environmental Report monthly to KNPC.
Play a lead role in case of emergency and liaises with the Site Engineer, area disciplines Engineers,
Site Supervisors, and HSE Engineer/Supervisor.
Safety Supervisor:
Assist Engineer in implementation of HSE policy, Plan & procedures at site.
Responsible for seeing hazards and unsafe work are indentified and mitigated
Participate in any investigation relating to HSE issues.
Verify that first-aid and emergency procedures are in place.
Comply with KNPC HSE requirements at project.
Manage and maintain the HSE-related records and documentation.
Verify that material safety data sheets (MSDSs) are maintained and communicated.
Through regular fieldwork, verify that all personnel are conversant with the HSE Management
System, its procedures, and related HSE instructions.
Participate and coordinate in making Risk Assessment.
Participate in monthly HSE meeting at site, KNPC HSE meetings as per requirement.
Conduct HSE training programs to all employees.
Conduct work place inspections /audits (Daily/Weekly/Monthly) with the area supervisor as
applicable.
Investigate and determine the cause of any accident or dangerous occurrence including near misses,
incidents etc., and ensure recommendations are complied to prevent the recurrences.
Ensure the availability of PPE and shall ensure that all site personnel.
Ensure that equipment delivered to site are inspected and verify that each equipment have a valid
Third-Party certificate and comply with KNPC requirements.
Inspect the fire extinguishers and ensure that there position are in easily accessible locations in line
with project HSE requirements.
Carry out safety inspection of equipment and tools.
Ensure work permit system being implemented and followed.
Ensure that good housekeeping is maintained throughout the period of work.
Ensure appropriate safety signs are in place and adequate

Familiar and comply with the safe work procedure, permits, HSE rules and regulations.
Identify unsafe acts /conditions and take corrective and preventive actions through job supervision.

Scope.
This procedure is applicable to all operations and functions. All project sites of Bader Al
Mulla have to proactively plan how to address occupational health and safety related
hazards and associated risks and environmental aspects and associated impacts for
exercising controls and preventive actions. These plans are developed in the form of
integrated HSE Risk Assessment Registers for all project sites to manage HSE risks in
line with client relevant procedures. This procedure is completely outlined according to
the guidelines given in the client's Risk Management procedures. Periodic review and
update of HSE Risk Assessment Registers shall be carried out based on following.
Change of management
Incidents (including health, safety and environmental)
Change in legal and other requirements
Successful completion of HSE Management Programs (HSEMP)
Consultation with workers and other interested parties like contractors and
visitors.

5.1 Hazard Identification & Risk Assessment Management.
Risk Assessment Registers are prepared incorporating hazard identification and risk
assessment inputs from workers and other interested parties like contractors and visitors.

This is done through KNPC risk assessment procedures. HSE Risk Assessment Register
lists potential hazards leading to ill health and human injury.
Following inputs are considered while identification of hazards:
Routine and non-routine activities.
Activities of all personnel having access to the place of work (including
contractors and visitors)
Human factors, such as behavior and capabilities
Identified hazards originating outside the workplace capable of adversely affecting
the health and safety of persons under the control of the organization within the
workplace
Infrastructure, equipment and materials at the workplace.
Modifications to the OHS management system, including temporary changes and
their impacts on operations, processes and activities
Any applicable legal obligations relating to risk assessment and implementation of
necessary controls, and
Design of work areas, processes, installations, machinery/equipment, operating
procedures and work organization, including their adaptation to human
capabilities.
Hazard Identification
One key element of effective safety risk management is a systematic approach to hazard
identification. The Hazard Identification methodology applied shall consider all the
factors involved in the system under study. This shall include all modes of operation
(routine and non-routine activities), emergencies, as well as external hazards.

5.2 Critical Task Risk Assessment (CTRA)
After the hazards have been identified, the risks arising from them are evaluated either
qualitatively or quantitatively. This typically requires carrying out a first stage named
"risk estimation", which entails assessing both the severity (consequence) and frequency
(likelihood) of hazardous events.
Critical Task Risk Assessment (CTRA) is a form of HIRA & DC, which is applied for
specific activities (maintenance, construction & non-routine activities) which are
identified as critical works.
The above-mentioned processes address a wide risk spectrum related to maintenance,
construction & non-routine activities. A combination of one or more of these processes
shall be applied for managing risks associated with such activities executed with work
permits. The processes which are used in addition to Work Permits will be explained in
the various sections of this document.
Categories of maintenance, construction & non routine work.
Maintenance, construction & non-routine works are categorized as given in the table. Work Permit is essential to carry
out all categories of works. In addition to work permit other tools such as Safe Work Practices Checklist & Critical Task
Risk Assessment (CTRA) are required for different categories of works as given below

CTRA shall contain only those hazards and control measures which are not addressed in SWP
Checklist and which could not be addressed in work permits
Manage the Safety Risk to acceptable level
After the risk has been identified and analyzed as per above, management decides what
corrective action (if any) is necessary to manage the risk to an Acceptable level.
Depending on the nature of the specific hazard(s), this may require different steps to:
Eliminate the hazard.
Prevent the hazards from occurring (eliminate) or to reduce the likelihood of the
incidents.
Reduce the consequences with other control measure.
Mitigate the consequences of the hazards which are inherent to the type of
operation or working environment and which cannot be prevented altogether and
have a chronic likelihood to develop into incidents with harmful consequences.
Recover from the consequences of incidents. Recovery and Damage Control measures
should be developed for all potential emergencies with the aim to prevent escalation e.g.,
from critical into severe or worse the results of the risk estimated as established above,
shall be compared to the KNPC Risk Tolerability Criteria (RTC), also given in the Safety
Risk Evaluation Matrix. The risk then shall be managed accordingly. This process is
called Risk Assessment, which refers to the process of objectively examining the risks
involved in a particular activity. The objective of the determination is to ensure that the
risks are fully understood.

5.4 Environmental Impact Assessment (EIA) Management.
Environment Risk Assessment Register is prepared incorporating environmental aspect
and impact assessment inputs from initial assessment review by environmental studies
and/tests including inputs from workers and other interested parties like contractors and
visitors. This is done through testing of air quality, water quality, noise quality, resource
saving potential indicators and waste management issues. HSE Risk Assessment Register
lists environmental aspects leading to impact on physical environmental performance
Definition: -
Environment Aspect: - An element of an organization’s activities, products or
services that can interact with the environment.
Environment Impact: - Any changes Any change to the environment, whether adverse
or wholly or partially resulting from an organization’s activities, products or services.
Emergency conditions: - Sudden, unexpected or impending situation that may cause
injury, loss of life & damage to the property, such as fire, leaks & spills.
Significant Environmental aspect: - An environmental aspect which has or can have a
significant environmental impact
Define Areas and Activities
In order to make aspect identification more manageable, the site activities shall be
assessed based on the area classification., provided all areas and functions are included

The aspects are identified as Normal/ Abnormal/ Emergency and also whether they are
regulated or not. The Evaluation of aspect/ impact is carried out by using the following

Environmental Aspects Identification
Identify the inputs and outputs to and from the site activities and then identify the
environmental aspects and impacts that arise from these inputs and outputs. This should
include (where applicable) identification of direct and indirect environmental aspects.
For each area and activity of the sites, there must be an ongoing process of aspect
identification that determines the current and potential impact on the Environment. These
include:
Emissions to air including Green House Gases.
Discharges to water or sewer.
Spills/releases to land/sea
Management/disposal of Hazardous and Non-Hazardous Wastes.
Water use.
Fuel and Energy use.
Use of Raw Materials and Natural Resources (Renewable/Nonrenewable).
Use of Chemicals.

If the impact of any activity is one or more of the following, then the impact becomes an
environmental emergency and the impact becomes significant. The emergency situations

Resource Saving Potential
If impact of any activity causes resource depletion and has a resource saving potential or
more, the impact becomes significant. The resource saving potential activities are as
follows:

Energy
Other Natural Resource
Identify the type of occupational health hazards (for e.g., chemical, physical,
biological, ergonomic, psychological and other related hazards) that are, or are
likely to be present in the work areas.
Identify the employees or group of employees who are potentially exposed to the
identified occupational health hazards.
Estimate the occupational health risks based on severity and likelihood of the
identified occupational health hazards.
Assigning significance of the Occupational Health Risks based on the
occupational health risk assessment matrix
Identify and implement the control measures for preventing the employees from
the exposure to occupational health hazards.
Comply with regulatory requirements related to occupational health, policy and
business requirements.

Environment Aspect register
The purpose of Environment Aspect Register is to identify those environmental aspects of
activities, products, and services that require management in order to:
Controlled Area: - is that area, in which KNPC exercise a control over all movement’s
maintenanace activities i.e., the area within the outside and inside fence boundaries or
open space where KNPC direct indirect control for operation reason.
Restricted Area.: - which area comes in the control area that means all fenced area in
which KNPC exercise a control over all movement’s maintenanace activities. Entry to the
restricted area is allowed only by an access pass (gate pass) which is to be shown at the
Security Check Point.
Hazards Area: - in the petroleum industry are classified according to the likelihood of a
flammable atmosphere being present. A flammable atmosphere has a concentration of
flammable gas, vapor, or mist, which is capable of being ignited.
Hazards area also divided in two parts
Division -01 which is normal area such as sampling point, and vent.
Division-02 abnormal Area such as Blast leak etc.
BMB shall comply with the applicable legal and regulatory requirement for the project. In addition to the
HSE requirements contained within the KNPC HSE Manual;
All the relevant and applicable national Kuwaiti laws and regulations not specifically referenced shall
apply.
Non-Kuwaiti regulations and standards such as OSHA standard and applicable ANSI standards
being industry accepted standards shall be relevant and shall be referenced as guidelines to carry
out the work in a safe manner.
Where statutory standards and/or regulations apply to the site the most stringent of these shall be
applied and used as the minimum requirement.
7. HSE COMMUNICATION:

7.1. Toolbox Talks:
Pre-Task Meetings:
The Job supervisor shall conduct a pre-task meeting before any work activity begins and discusses the
completed pre-task planning and documents with all employees directly involved in the activity.
HSE training is considered as paramount in the BMB HSE Management System. A trained employee is
an asset to BMB, who plays a vital role in prevention of accidents at worksite. The need and commitment
of giving training to employees has been emphasized in the company HSE policy.
Safety training shall be imparted to the employees as per KNPC Training and Orientation.
HSE Orientation:
All BMB employees shall attend the KNPC Orientation/Induction training provided by the KNPC HSE
Department. Further training needs shall be determined in accordance with an individual’s changed
responsibilities, changed duties, or changed location by the Project Management.
Certified Training:
BMB Project Management shall ensure certified training of each trade / craft / category of employee
provided through a KNPC approved Third Party / Agency and shall include, but not be limited to the
following:
BMB project management shall plan the mobilization of man power and accordingly HSE training
Schedule shall be prepared and submitted to KNPC for training arrangements.
Records are required to support activities of Contractor HSE programs for both control and audit
purposes. Records that shall be maintained and retained within the contract duration at the job site
should include the following:
(a) First aid log of all first aid cases reported and treated, showing date, name of
injured, job nature of injury and type of treatment given.
(b) All incident investigation report (i.e. injury, illness, traffic, property damage,
environmental incident and as well as Near Miss Incidents).
(c) HSE meetings or toolbox meeting records or minutes showing date of meeting,
who attended, the subjects discussed and who conducted the meeting.
(d) Job site inspection / Tool Inspection / audit reports and status of the action plans.
(e) Records to show dates, name of participants and subject of training programs.
(f) Lifting tackles and crane or equipment inspection records, 3rd party certificate.
(g) Infringement Reports and traffic violations
(h) Maintain all EMS required documents & records such as Environmental Aspect
Register, Environment Incidents Reports, ISO 14001 Audit Reports, Environment
Inspection Reports, Waste Transportation Manifest Records, Contractor
Environmental Monthly Reports, EMS Training Records, EIA Study
Recommendations, etc.
(i) Work Permit Risk Assessment
(j) Maintain Contractor’s OHS Risk Register (i.e. in addition to KNPC Custodian OHS
Risk Register) and training records for OHSAS awareness to Contractor employees
(k) Medical Examination Records
(l) Monthly KPI Reports
8.7. Records:
Training records such as attendant sheet shall be maintained at worksite by safety Engineer/Supervisor.
These records shall be made available during the HSE audit and as and when required by KNPC.
Safety personnel shall motivate the employees and based on the performance shall recommend for other
awards like Safe Driver, Best near miss reporting.etc.
Disciplinary Observations:
BMB shall follow the KNPC shall implement a fair disciplinary process based on the accepted “Balance
of Consequence” principle, in line with the KNPC Penalty System for HSE Violations’.
Employees shall be made aware about the KNPC warning system. The first warning shall be verbal, the
second warning shall be written and the third (and final) warning shall result in the immediate removal
from site.
As per KNPC procedure, to indicate an individual has been warned, their Contractor Site access card
shall be punched with one hole for each warning. On the third warning, the card shall be withdrawn and
the responsible individual removed from the site.
9. OPERATIVE CONTROLS
In case of an emergency, work shall be suspended and validity of any work permit
ceases in the affected area. Executor or his crew shall immediately stop work
when a fire or emergency alarm is sounded or accident of any kind occurs. The
work permit shall be renewed to resume the work after the fire or emergency is
over.
Issuer, his deputy or KNPC Safety is authorized to suspend work and withdraw
the work permit, if the permit conditions or safe work practices are violated and/or
if the site conditions have changed leading to a hazardous situation, and shall
immediately inform the reasons for stopping to the Issuer/Executor or their
superiors.
If work is not started or is suspended due to safety reasons after a work permit has
been issued, the work shall only be started or resumed after establishing safe
conditions and renewal of the work Permit

Once schedule work is complete, the permit to work certificate will be closed and
returned to the originator.
Both the copies of electronic work permit shall be signed by Field Operator and
closing to be done electronically in the system by the Executor and the Issuer. In
case of manual permits, Executor, Field Operator and Issuer shall fill and sign
Section-8 (permit closure) on the "Original" and “Field Operator Copy" of the
work permit at the completion of work or the end of last renewal whichever comes
earlier (closing Issuer & safety copy is optional in case of manual work permits).
Copies of different types of permits shall be filed separately in chronological
(date) order in box files for future reference for at least one month (30 days) from
the date of issue/closure. This is applicable for both the copies of electronic work
permits and F.O. & Executor copies of manual work permits.
“Executor copy” which belongs to Executor shall be filed and available with the
KNPC /Contractor supervisor in charge of execution of the job.
Field Operator copy” shall be filed and available at the issuing office (filing or
retaining issuer copy, in case of manual work permits is optional).
In case of manual permits, “Safety copy” which had been dropped in the permit
box by the issuer shall be collected by the Area Safety Engineer every workday
and filed.
BMB will ensure that the issued work permit copies will be maintained by the site
permit executor on schedule work is complete.
When permit is closed BMB will ensure that all equipment’s are stopped, electrical system
isolated, all hot area cool down and any spark / molten metal extinguished. Access will not be
blocked by equipment of maintenance, barricading installed and warning signal will be
provided on open area and ensure proper housekeeping before closing the permit.
Compliance of work permit audit shall be done by the safety supervisor, as Refer to work permit
Audit Check list Appendix-10

BMB shall comply with the requirements of KNPC - Confined Space Entry
A Confined Space Data Sheet must be developed, approved by the Confined Space Entry Authorized
Permit Issuer, checked by another person before a Confined Space Entry Permit is approved, and
filed in the BMB and KNPC Office.
The Confined Space Data Sheet Guidelines/Checklist shall be used to prepare the Confined Space
Data Sheet. A cold work permit should be obtained if non-sparking hand tools are being used in a
Confined space and a Hot Work Permit if powered tools or mechanical equipment are being used
No person should enter any confined space unless a confined space entry permit is obtained. This
permit is an authorization only for personnel entry.

Work within the confined space will require additional documents in conjunction with the confined
space entry permit, such as, hot or cold work permit, excavation authorization, etc.
Where atmosphere within confined space is initially made safe, but there is a reason to believe that it
may become unsafe during the period for which entry is authorized, e.g., from emission of fumes from
sludge or deposits contained in the space or welding fumes or inerting, in such case continuous gas
monitoring is required.
Gas monitoring should be carried out by KNPC authorized gas tester using multi gas detectors
approved by KNPC.
Periodical calibration of the gas monitors shall be ensured
Gas tests are mandatory for confined space entry. Continuous gas testing shall be carried out to
identify the percentage of toxic and hazardous gas.
Minimum requirement should be O2 %, LEL% and H2S ppm & CO ppm once at the beginning of
shift/work.
Adequate illumination should be provided using 24 Volt or below explosion proof lamps. Use of any
electrical equipment above 24V should have earth leakage tripping device and KNPC Safety approval
Mechanical ventilation equipment should be properly grounded (earthed) to dissipate any static
charges.
PPE for a confined-space entry shall be identified based on an assessment of the hazards associated
with the material, atmosphere, conditions that are normally in the space and the work being planned.
BMB shall ensure that employees working inside confined space must use the PPE as identified and
specified in the confined space entry permit.
Equipment MUST be grounded where static electricity is a potential hazard or could be created by the
activities in or around the confined space
All personnel involved with confined space entry should be trained in confined space hazards &
rescue.
9.2. Excavation:
BMB shall follow KNPC Procedure - Excavation, Trenching, and Shoring for excavation activity.
No excavation work in KNPC area shall be taken without ensuring KNPC approval for the job for that
location.
An excavation permit will be issued by an KNPC Excavation Authorized Permit Issuer for all
excavations
Excavation notification is required for all excavations regardless of its depth.
A hot work permit shall be required for any excavation work involving mechanical digging or
excavator.
Excavations and trenches shall be inspected daily and after every rainfall to determine if they are safe
to continue construction work. As well as before starting the work.
Before start of any excavation, the location of underground obstructions shall be investigated and
confirmed by the responsible authority as applicable.
The required PPE must worn by the worker involved in excavation activities.
Ladders shall be provided every 25 feet of lateral travel in the trench. Ladder shall be securely
supported at the bottom as well as the top.
Excavations such as ditches, trenches or holes shall be sloped sufficiently to prevent cave-ins or
slides. The angle of repose should be not less than 45°.Personnel removing shoring after completion

of work shall not be in the bottom of the excavation. Shoring shall be removed in a manner to prevent
cave-in on personnel.
Barricades, handrails, signals or other appropriate warning devices (tape) shall be provided to protect
the employees.
Ropes, chains and other tripping hazards shall be sufficiently marked and color-coded to be clearly
visible during the night time. Excavated cables, pipe or other underground obstructions shall be
supported where required in order to avoid breaking, bending or damage.
Groundwater shall be removed from excavated trenches when required and/or applicable.
Excavated material shall be placed at a distance not less than 0.6 meter from the edge of the
excavation.
All equipment, materials and plant must be kept clear form excavations to prevent accidental fall. Any
heavy equipment shall be kept clear from the excavation area by at least 3 meters away.
All trenches and excavation 4 feet (1.2 meters) or deeper shall be assessed regularly for designation
as a confined space in accordance with Procedure Confined Space Entry.
Lifting or Rigging activity of any size or reach on the projects shall be properly planned and carried
out. Lifting operation shall not start before the lift plan is approved.
BMB shall follow all statutory and contractual requirements during planning and carrying out all lifts.
BMB shall follow KNPC Safe Work Practices on Lifting Appliances & Tackles and KNPC Procedure,
Cranes and Rigging Equipment and Aerial Lifts, Elevating Work Platforms, and Material/Personnel
Hoists.
The lifting activity shall be carried out by authorized and trained persons under supervision.
Before each lifting work, all persons who are engaged in that work should be aware of the movement
of material and the equipment.
The operator of lifting equipment shall inspect the equipment for safe operation every day prior to
start of the work based on the check list.
Operator shall not start the operations without valid permit.
Certified lifting material (ropes, buckles, nylon slings, wire rope slings) shall be used for hoisting
operations. Faulty material shall be removed from the work site.
Necessary warning boards shall be displayed and the total area has to be barricaded to avoid entry of
other persons.
Rigger / Banks man shall be available at all lifting activities
The signal shall be given only by Rigger / Banks man
Shall wear the fluorescent jacket for clear visibility
Proper communication shall be maintained with operator
Visual inspection of tools and tackles shall be carried before start the lifting.
During the project execution, various types of mobile equipment and vehicles are used. These mobile
equipment posses various hazards, which can lead to injury or property damage.

BMB project management shall ensure that the mobile equipment and vehicles utilized for the project
shall meet the KNPC requirements. The general requirements are as follows but not limited to,

Have a valid Kuwait driver’s license or certification with the appropriate statutory endorsement.

Drivers/operators may also be required to attend additional training or successfully pass a skills
test/performance evaluation.
o Personnel who drive motor vehicles must also obtain a site permit or be designated to drive
o Drivers must be familiar with the Kuwait, KNPC traffic rules and regulations and also site traffic
rules.
Operator/Driver shall be
o Be mentally alert and possess the physical ability to drive a equipment & vehicle safely;
o Display a responsible attitude;
o Be familiar with the motor vehicle to be operated.
o Provide evidence of training, qualifications and/or certifications received from a KNPC approved
third party agency.
Only trained and authorized operators shall be permitted to operate the designated equipment.
BMB project Manager is responsible to allocate resources required to manage and support the overall
motor vehicle safety program.
Falls from height are responsible for many serious and fatal injuries. Work at height generally involved
working on scaffolding, mobile elevated working platform, ladders etc
Working at Height shall be
o Properly planned
o Appropriately supervised
o Employees should be competent or trained
If work at height cannot be avoided, a risk assessment should be carried out as per KNPC procedure.
If weather conditions jeopardize health and safety work at height should be stopped
Proper fall protections like guard rail system, personal fall arrest system shall be provided
The work at Height activity shall be carried out under supervision only.
BMB shall comply with the KNPC - Fall Protection procedure.
9.6. Scaffolding:
Scaffolding includes tube and couplers, modular frames, drop staging, swing stages, purpose design
stages all with full decking, guardrails, and toe/ fender boards.
Scaffolding in light, medium, and heavy-duty applications must conform and be constructed in
accordance with KNPC site procedures.
Employees working from swinging scaffolds or boatswain’s chairs must use Personal Fall Arrest System
where the lanyard or lifeline is attached to an attachment point independent from the scaffold or chair.
Employees involved in the erection or disassembly of a scaffold will use fall protection when appropriate.

Electrical hazards differ from other hazards found in construction work. The great majority of electrical
incidents result in shock burns or even death. Fire and explosion from sparks in flammable atmospheres
can and do lead to loss of life and serious damage to equipment, materials or producing facilities. The
following are the causes of the Electrical hazard,
All electrical work must be accompanied by an appropriate work permit and the procedure for lock out
/tag out for safe execution of work.
Conductors and parts of electrical equipment that have been deenergized must be locked out and
tagged out, tested to verify zero energy in accordance with Procedure Hazardous Energy Control,
before any work may commence.
Only authorized person shall perform the repair works.
Employees using electrical appliances shall be cautious and alert at all times.
All electrical appliances shall be operated as per equipment operation manual.
All electrical equipment must be properly grounded. Temporary wiring installations and portable
electrical hand tools shall be provided with ground fault protection .
GFCI protection is required for all portable hand tools used inside a vessel or in wet areas.

Ground fault circuit interrupters or Earth Leakage circuit Breakers shall be provided.
Workers shall wear required PPE.
The presence of underground electrical cables must be determined before any excavation work is
taken up.
Keep the area around electrical equipment as dry as possible the use of duck boards should be
considered on wet floors
An electrical fire, if it occurs, shall be put-off only using a CO2 or DCP extinguisher. Water & foam
extinguishers shall not be used.
Electric power hand tools must have the safety device
Lock out /Tag out program is intended to prevent unexpected energization, startup, and uncontrolled
energy releases, which could cause injury to persons and/or damage to equipment. BMB shall follow that
the employees involved in the performing or supervising work on systems / equipment shall be trained in
the practices of LOTO procedures. BMB shall follow the - Hazardous Energy Control procedure while
performing activity on system or equipment.
All operations involving welding cutting and Burning shall be carried out in accordance with - Hot Work
(Welding, Cutting, and Burning) in addition to Permit to Work Procedures, as required. The main hazards
associated with welding and cutting activities are-
Fire or Explosion
Electrical shocks
Failing gas cylinders
Flying particles like weld bead, metal piece
Welding fumes
Radiation from UV and infra red
Noise hazards during weld preparation and some of the welding activities
Following General guidelines shall be followed during the Welding, cutting and Burning activity:
All hot work shall be done by qualified and authorized person only.
All Welding equipments shall be properly maintained and regularly inspected.
Welding work to be done in a restricted area only after obtaining Hot Work Permit if required and all
requirements pertaining to Hot Work Permit shall be complied.
Welders shall not work alone.
Welders’ crew shall be trained on Fire Fighting.
Fire Watcher and fire extinguishers shall be kept ready at the worksite.
Forced ventilation shall be arranged during welding work in a confined space.
Housekeeping shall be always maintained at the worksite. Working area shall be free all flammable
materials including oil, grease cotton waste etc.

Used welding rod buds etc., shall be collected and put it in proper waste bin; it should not thrown
around.
Always maintain the cable management at the worksite to avoid the prevention of electrical hazards.
Welding or cutting torches and hoses shall not be connected to cylinders when stored in any
enclosure. When there is no work and equipment is unattended, all valves of the gas and oxygen
cylinders shall be closed.
Prior to carrying out any welding or cutting operation, the equipment or pipeline must be positively
isolated, depressurized, decontaminated, purged or flushed in compliance to work permit requirement
for the job.
Compressed gas cylinders shall be handled with care, properly supported in an upright position away
from any source of heat or flame and securely tied-off. Cylinders not in use shall have the protective
cap in place, stored vertical and outside the work area.
Flashback arrestors shall be used behind the reducing valve on acetylene & oxygen cylinders.
Caution shall be taken during overhead cutting, burning and welding operations to safeguard
personnel and prevent falling sparks.
Welding blankets and fire extinguishers shall be readily available or plant or facility fire hoses
attached to existing firewater hydrants are ready for use.
Welding cables and compressed gas cylinder hoses shall be inspected regularly.
The ground cable shall be attached as close as possible to the work piece, by means of a clamp.
Concrete reinforcing bars shall not be used as grounding.
Welders and welder helpers shall appropriate PPE.
When not in use welding machines shall be turned off. Welding machine shall be stopped while
refueling.
Hoses and cables shall be kept clear of passageways, ladders and stairs.
When welding, burning and cutting are performed in a workshop or other enclosed building adequate
ventilation shall be provided.
Compressed gases have very high-risk hazards. The gases are contained in heavy, highly pressurized
metal containers; the large amount of potential energy resulting from compression of the gas makes it a
potential for mechanical and chemical hazards.
BMB shall follow the following KNPC procedure for procurement, transportation, handling and storage of
compressed gas cylinder.

Safe Work Practices on Compressed Gas Cylinders
BMB shall ensure that the CG cylinders meet the requirement of KNPC.
All gas cylinders shall be color coded and all the personnel must be aware of the applicable colors.
Valve protection caps shall be in place and secured.
When cylinders are hoisted, they shall be secured on a cradle, sling board, or pallet.
Cylinders shall be moved by tilting and rolling them on their bottom edges. They shall not be
intentionally dropped, struck, or permitted to strike each other violently.
When cylinders are transported by powered vehicles, they shall be secured in a vertical position.
Valve protection caps shall not be used for lifting cylinders from one vertical position to another.

Unless cylinders are firmly secured on a special carrier intended for this purpose, regulators shall be
removed and valve protection caps put in place before cylinders are moved.
A suitable cylinder truck, chain, or other steadying device shall be used to keep cylinders from being
knocked over while in use.
After completion of work, when cylinders are empty or when cylinders are moved at any time the
cylinder valve shall be kept closed.
Compressed gas cylinders must be secured in an upright position at all times.
Empty cylinders must be kept separate from full cylinders with a sign board of full and empty.
Oxygen cylinders in storage must be separated from fuel-gas cylinders or combustible materials
(especially oil or grease), a minimum distance of 3 meter.
Cylinders shall be stored in a well-protected, well-ventilated, dry location.
Appropriate number of fire extinguisher shall be kept near the storage area.
All hose in use, carrying any flammable gas must be inspected thoroughly at the beginning of each
shift.
Hose which has been subjected to flashback or severe wear or damage or in doubtful condition must
not be used.
Hoses and cables must run in orderly fashion and must not block access ways.
Eyes and ears must be always protected when blowing gas to clear hose or nozzles.
Unattended or not in use hose must be depressurized.
At the end of shift hoses and torches must be withdrawn from worksites and coiled neatly besides
cylinders.
It is the responsibility of all to maintain the fire extinguishers by checking at regular intervals. Safety
representative shall maintain the record of the same.
Accumulation of trash, rubbish, oily rags, combustible materials or fire hazards of any nature shall not
be permitted.
Areas around welding and cutting operations shall be kept free of flammable materials of all nature
(wood, paper, flammable liquids, etc.).
Fire blanket shall be used for hot work activity.
Flammable or explosive type materials such as fuel, oil, cleaner, etc. shall be properly stored, clearly
identified, separated in a dedicated area and kept clean of work areas. Warning signs shall be
provided in such storage areas.
Welding or flame-producing operations shall be stopped within a radius of 10 meters during any
refueling operations.
Location of fire extinguishers shall be clearly marked and free access to be maintained.
Prior to leaving the worksite employees shall ensure that all electrical equipment is switched off.
Electrical equipment shall be adequately protected against overheating.
Smoking shall be allowed only in designated areas.

All welding and cutting torches must be equipped with flash back arrester.
Refueling of equipment shall be done only in prescribed areas and with approved fire fighting
equipment. Stop the engine while refueling.
Employees shall take all measures to minimize spills and to clean up immediately and spills, which
may accidentally occur.
There must be a fire extinguisher, water hose or other fire control equipment easily accessible for
each welding, cutting, burning or other such operation.
Every fire incident shall be reported immediately whenever they occur. Emergency telephone
numbers of departments and responsible to be contacted shall be available and posted in accordance
with the contractual requirements.
9.12. Tools and Portable Power Tools:
Hand tools are those tools for which the hand provides the motive force, such as hammers, picks,
shovels, screwdrivers, chisels, wrenches etc. power tools are actuated by power source such as
electrical, hydraulic ,pneumatic etc. Incidents arising from the use of hand tools are frequently
caused by human error; poor quality tools etc.
BMB shall adhere to KNPC Hand and Portable Power Tools procedure, for the selection, use, and
maintenance of hand tools and portable, power-operated tools.
General Requirement:
All hand and power tools used on the project shall be maintained and operated in a safe
condition/manner.
No modifications or additions that affect the capacity or safe operation of tools will be made
without the manufacturer’s written approval or approval from a registered professional engineer or
the KNPC Site HSE.
Good and proper quality of hand tools shall be provided for all jobs.
The correct type, size, and weight of tool shall be used.
Tools shall be regularly cleaned and inspected before use.
Employee shall wear appropriate PPE as per the hazards.
If damage is detected that shall be immediately repaired or remove from the site. Proper storage
facilities shall be provided.
Tools shall not be left lying around the job where they could be damaged.
Rotating tools should be switched off and held until rotation has stopped before they are laid
down.
Employees keep hands and other parts of the body clear of the point of operation (blade, bit, etc.)
at all times while the tool is connected to its energy source.
Training shall be given to employees for the rotating tools, changing blades, cutters, discs etc.
Grinding machines shall be handled with care.
Grinding wheels shall be checked to make sure no defects are detected.
The proper size and type of wheel shall be fitted to the tool so that the maximum permissible
running speed of the spindle does not exceed the maximum periphery speed displayed on the
grinding wheel.
All personnel using abrasive wheels shall be protected by eye protection equipment.
All portable power tools shall be equipped with spring loaded switch (dead man switch) which will
actuate only when pressed. The switch shall be free from any locking arrangement.
Only authorized and trained personnel shall operate the rotating equipments. Machine guards are
provided to protect employees from the rotating parts of machine.
Employees shall not alter, modify or remove any machine guard. Finger rings, bracelets, necklaces,
and jewelry shall not be worn while working on rotating equipment.
All vehicles and equipment, regardless of type or classification, shall be inspected daily by end user
prior to work commencing by or upon shift hand over by the respective operators. Daily inspection
reports and checklists shall be maintained with the vehicle.
Each employee is responsible for keeping his/her work area organized and uncluttered – and to
prevent trash, waste materials, and debris from accumulating. Those employees doing work in
multiple areas are responsible for cleaning up after they complete that job. Each craft personnel
are responsible for cleaning-up their work area. No job is complete until the area is cleaned-up.
Housekeeping shall be done continuously and regularly.
Job Supervisors are responsible for housekeeping and shall periodically inspect each work are to
ensure that the housekeeping expectations are being met.
Housekeeping must be maintained on a daily basis; cleaning and sweeping will be done in a
manner that minimizes the contamination of the air with dust or particulate matter.
Littering anywhere KNPC property is prohibited, and subject to disciplinary action.
Floors, working surfaces, and passageways must be kept free from protruding nails, splinters,
loose boards, and openings.
Hoses and electrical conductors that cross aisles or passageways must be covered or suspended
overhead so that they pose no tripping hazard.
Exits, fire alarm boxes, fire extinguishing equipment, electrical panels, and other emergency
equipment must be kept clear of obstructions at all times.
Containers shall be provided for the collection and separation of waste, trash, oily and used rags
and other refuse.
Garbage and other waste shall be disposed off at regular intervals in a manner approved by the
KNPC.
BMB shall comply with KNPC - Barricades, Signs, Tags, and Lighting Procedure for the setup,
maintenance, and uses of HSE barricades, signs, and tags.
BMB Project Management shall ensure that areas have sufficient signage as required by the Contract or
as defined by KNPC. Signage shall be visible at all times where a hazard exists. This shall require the
use of illumination during periods of poor visibility (nightfall and dust storms, etc.).
Employee failure to heed safety signs, tags, or barriers will result in appropriate disciplinary action.
Barricades, including barricade tape shall be accompanied with signs or tags to clearly communicate the
hazard and include the following:
Name and contact details of the supervisor/foreman who placed the barricade
Purpose or reason for the barricade (the hazard[s])
Date placed
The standard of barricades used on the site shall be determined by the period, purpose of use and
hazards of the activity. The barricade tape type is specified in KNPC procedure shall be followed.
PPEs shall be provided to all employees as per the KNPC- Personal Protective Equipment Procedure
Mandatory PPE’s include Safety Helmet, Safety Goggles, Safety Shoes, Gloves, Ear Plugs and Coverall.
The coveralls shall be provided with BMB logo. Any additional requirement of specific PPEs shall be
evaluated based on the hazards as per the job at worksite. The employees shall wear the recommended
PPE at the worksite as specified in the work permit, work procedure. Project Management shall ensure
that BMB visitors shall wear approved PPEs only.
Samples of PPEs shall be submitted for KNPC approval and only approved PPEs shall be used at the
worksite. All PPE shall be compliant to ANSI / OSHA Standards and conform to the latest “KNPC PPE
Specifications” All PPE shall conform to the latest “KNPC PPE Specifications” mentioned in KNPC HSE
document “Personal Protective Equipment Program”.
Job Supervisor shall ensure that required PPEs are used by employees at worksite and Safety personnel
during their daily safety inspection shall also ensure that all employees are adhered to the required PPEs.
Safety personnel shall give training on the PPE requirements, use and maintenance of PPE through
orientation and training, as well as in the safety task assignment (STA).
Employees must inspect PPE before use to ensure it is fit for use. Defective or damaged PPE must be
immediately removed from use
PPE must be stored in a designated area out of the elements (heat, light, moisture, etc.). Project Site
Stores shall maintain a register of PPEs issued to the employees. They shall also monitor their
requirement, consumption and take necessary prior action to ensure required PPE is available at
worksite.
The damaged or deteriorated PPEs shall be discarded immediately from the worksite. PPE contaminated
with toxic substances should be disposed as toxic waste as per KNPC waste management plan.
Damaged or out of service PPE should be discarded as solid waste.
In case of an emergency, reporting the incident, alerting the personnel and following the instructions by
EMP team shall be done as per KNPC Emergency Preparedness and KNPC Emergency Response plan.
During project emergency or natural disaster, BMB and its employees shall follow KNPC provided
Emergency Management Plan for evacuation of the work area. BMB project management is responsible
for advising its employees the potential hazards, emergency signals, and primary evacuation routes and
processes. Project Management shall appoint the appropriate personnel to perform emergency functions.

BMB shall participate in total site emergency evacuation should it be deemed necessary due to the nature
of an emergency situation.
Stop all work immediately and ensure that all equipment is left in a safe condition.
Heavy equipment shall be left out, properly at site so that they don’t obstruct emergency
operation or hamper evacuation.
Leave the work area, go across wind and assemble at an assembly point (look at wind socks
or drifting steam for general wind direction).
Do not panic, keep calm. Do not run push or overtake.
Keep noise to a minimum, so that instruction can be hear clearly.
Remain at Assembly Area for headcount.
Unauthorized personnel may not re-enter the emergency area for any reason until the “all
clear” signal is sounded.
Do not make unnecessary telephone calls during an emergency.
Do not re-enter area or re-start work until positively informed by the KNPC that it is safe to do
so.
Boards with emergency contact no and name of contact person shall be displayed at worksite.
Project Manager shall ensure the accounting of their employees following an emergency evacuation at
assembly and report any suspect missing persons to KNPC immediately.
Project Manager:
Play a lead role in case of Emergency Evacuation as per KNPC procedures.
Coordinate with KNPC emergency response team.
Communicate job/safety supervisors in case of evacuation.
Coordinate in the mock drills organized by KNPC.
Job Supervisor:
Follow instruction from site In charge or KNPC incident controller.
Alert all personnel under his supervision.
After instruction, ensure all jobs are stopped and ensure that all equipment is left in a safe
condition.
Safe evacuation of employees to transfer/gathering point.
Coordinate with safety personnel during emergency situation.
Participate in mock drills.
Safety personnel:
Communicate emergency to all employees.
Guide all persons to transfer point/ gathering point.
Aware all persons on emergency preparedness through tool box talk.
Be informed about transfer/assembly point, emergency routes and emergency procedure.
Coordinate in the mock drills organized by KNPC.
Employee:
Follow instruction of incident controller.
Stop all activity safely.
Proceed to transfer/gathering point as per instruction.
Resume duty after all clear declared by incident controller.
Participate in mock drills.
Any incident including Injury, occupational illness near miss, damage to property, motor vehicle accident
or harm to environment if occurred, it shall be reported to KNPC HSE Representative immediately upon
detection and submit a documented investigation report within 24 hours of notification..
All investigation(s) reports and support documentation shall be submitted to the KNPC HSE Manager.
BMB shall complete and retain on file all “Employer’s First Report of Injury” and incident investigations,
and shall provide KNPC copy within 24 hours of event.

All personnel are responsible for reporting incidents (including near misses) and hazards to their
immediate supervisor as soon as practicable after the incident occurs or the hazard is identified.
Whenever practicable, every individual is required to immediately rectify identified hazards provided they
do not expose themselves to any danger.
Engineer/Supervisor shall ensure reporting of incidents as per procedure and shall take appropriate
action to minimize and control loss.
KNPC Form shall be used to document any incident and it should be submitted within 3-5 days to project
and HSE representatives according to KNPC Incident Reporting and Investigation.
The incident shall also be reported to BMB Corp-QHSE within 24hrs as per the BMB procedure in the first
information report.
Investigation:
Incidents shall be subject to a detailed investigation dependent upon severity and required reporting. The
report shall meet KNPC reporting requirements.
The extent of investigation should be commensurate with the potential severity of the incident. Incidents
and near misses that had the potential to result in severe injury/damage should be thoroughly
investigated.
The HSE Representative should conduct the investigation together with the supervisor(s) of the
individual(s) involved in the incident. All investigation(s) reports and support documentation shall be
submitted to the KNPC HSE Manager as per - Incident Reporting and Investigation.
BMB Site HSE representative submit the Investigation report to BMB Corp-QHSE as per the BMB
Incident/Accident Reporting & investigation procedure.
BMB shall comply with the corrective and preventive actions suggested to prevent incident recurrences in
the investigation report. All the incident reports and investigation reports shall be documented.
Based on the accident/incident investigation reports and analysis, Safety Flash like HSE Alert shall be
circulated. It is a method to share lessons learned from incidents. It explains briefly about the incident,

root causes, precautions and safe work methods to prevent reoccurrence of incident. This method helps
to educate and encourages the employees to stop the reoccurrences of incident.
HSE alert shall be communicated to all employees through Tool box talks and also it shall be displayed at
the site office.
The BMB Project Manager shall ensure that the monthly safety performance statistics report shall be
completed and submitted to the KNPCHSE Manager, or designee, on a monthly basis, in accordance
with the KNPC HSE Reporting Schedule. The closeout for KNPC reporting is the last Friday of each
month.
Monthly report shall cover HSE performance for the period covering a summary HSE activities, areas of
concern together with corrective action plans, third party/government visits, audits, summary of
environmental performance, graphical analysis detailing leading and lagging KPIs, man-hours, recordable
injury and lost work day injury rates on both a cumulative and twelve (12) month rolling basis.
BMB Management has overall responsibility for execution of the KNPC health management procedures.
Project management shall include health risks controls in the HSE plan, procedures, method statements
and Job Safety Analysis to mitigate against potential health hazards and to provide that sufficient
equipment and resources are available to effectively execute the requirements of the KNPC health
management procedures.
BMB shall comply with Industrial Hygiene Program procedure for anticipating, recognizing, evaluating,
and controlling exposure to chemical, physical, and biological agents in the workplace.
BMB shall provide KNPC approved First Aid Supplies and ensure these are readily available to their First
Aid Personnel during the performance of the project work. The First Aid Box shall be placed in a flexible
point at worksites within the reach of workers.
BMB will provide Training for Selected Employers as First Aider and they will be coordinating regarding all
first aid activities required during the project.
General Health and Occupational Health of employees are considered very important in Health, Safety &
Environment Management System. Employees shall be medically examined for their fitness and also for
their respiratory fitness as per the KNPC requirement.
Project Management ensures that all employees shall undergo pre employment and periodic medical
examination as per KNPC guideline. During the execution of project employee working in confined space,
shall undergo medical examination fitness test and medically fit employees are allowed to work inside
confined space. Safety Supervisor shall make schedule for the medical examination as per the KNPC
guideline and coordinate to conduct these medical examination with Corp QHSE dept.
All the medical records shall be maintained at site and shall be submitted to KNPC if required during audit
or when ever asked.
As per the Kuwait State law drugs and alcohol are prohibited. Any case of employees found involved in
any illegal activity like consuming or trading Drugs and Alcohol shall be dealt with Kuwait State law.
Smoking shall only be permitted in designated smoking areas. Each designated smoking area shall be
authorized by KNPC. Smoking outside these designated smoking areas shall result in severe disciplinary
action, including immediate removal from the site.
A Site or Area wide no smoking policy may be implemented during phases when combustible material
such as hydrocarbon based products have been introduced. Smoking tobacco is prohibited in vehicles
and equipment associated with the project site. Smoking shall not be permitted during emergencies.
Carrying of matches and lighters into the Hazardous Areas is prohibited.
All the employees make aware about the drugs, alcohol and smoking policy during induction training and
regularly during tool box talk.
Heat Exhaustion:
Heat exhaustion can occur if a person is exposed to hot weather conditions, expends too much energy,
and does not drink enough water / fluids.
o Help the victim to lie down under the shade or in a cool area.
o Give water in small amounts every 2 to 3 minutes until the victim recovers.
o Call by radio for medical assistance from nearby clinic / hospital.
o DO NOT give ice to the victim.
o Apply cool water to the back of the neck and wrists.
o Muscular cramps might occur as a result of salt depletion. In such cases, increase the
salt intake of the victim.
Heat Stroke:
Heat stroke is very rare but is potentially life threatening.
Heat stroke may occur after heat exhaustion has set in and has the following symptoms:
There are many areas of construction work where transport of material is manually done. It is noticed that
majority of injuries occur in such areas. For example the results of lifting incorrectly will results in back
problems, slipped discs and a complexity of internal injuries. It is therefore essential that the correct
methods of lifting are used.
Four significant points shall be observed during lifting,
It should be noted that for manual handling employee should know their limits and ask for assistance with
loads beyond their capabilities. Loads, even when properly lifted, can slip and lifting should be prepared
for such an eventuality. Proper personal protective equipment shall also be used.
A systematic and structured audit is a strong tool in effective implementation of a Health, Safety and
Environment Management System. Regular work place safety inspections and audits give the status of
compliance of safety at work place. It gives an opportunity to identify the compliance on safety at worksite
and to improve continually. Therefore, a detail system of auditing shall be followed during the execution of
contract.
Daily Inspection:
Site Safety Personnel shall inspect the worksite during his daily visit with respect to HSE issues. The daily
safety inspection by Safety Personnel shall cover:
Any deviations, observations for improvements shall be communicated to the Site Incharge for
rectifications. The observations with corrective actions shall be recorded by Safety Personnel.
Inspection of the work area shall be conducted by Job Supervisors prior to the start of work on a daily
basis as part of the STA [Safety Task Assignment] program requirements.
Weekly BMB HSE inspections shall be conducted by the area responsible HSE Representative and
Supervisor on all site areas and recorded on a KNPC approved format.
Records of weekly inspections shall be maintained by BMB and held available for KNPC Audit.
The BMB HSE Manager/Engineer shall develop an audit schedule as part of their Site Specific HSE Plan,
and during the term of the project, conduct regularly scheduled project/site audits of HSE Management
System.
The audits shall address both KNPC and BMB organizations and system compliance. Audit reports shall
be held available for KNPC Audit and subject to KNPC approval.
The scope of the audits shall include all work areas (including site offices and facilities) and cover:
Project/site HSE management system
Control measures for potential major hazards
HSE records
HSE work methods and procedures
Workplace inspections and follow up or actions
Audits shall examine the continued suitability of the project’s HSE management system procedures and
associated plans, and the effectiveness of their execution. The audits shall be conducted and reported in
writing in accordance with the project procedures -Inspections/Assessments and Audits.
The Audit report shall be submitted to Site Incharge/Project Manager for the compliance of the
recommendations. After the compliance of recommendations Project Manger shall submit the report to
Corp-QHSE for the closing of audit report.
The HSE management system will be reviewed by the BMB Project Management at planned intervals, to
ensure its continuing suitability, adequacy and effectiveness. BMB Management Review Procedure will
be followed for management review meetings. Inputs from the project to the management will include the
