I, ROMMEL GALLINERA GAMBA, Filipino, of legal age and a resident of

128 Jade St, Malanday, Marikina City, after being sworn to in accordance with law,
hereby depose and state that:

1. I am a holder of Company ID issued by Job On the Go.

2. However, said Company ID was lost/misplace sometime on August 15,
2023.
3. Said Company ID are now beyond recovery and is considered lost for legal
purposes.
4. I am executing this affidavit to attest to the truth of foregoing facts and for
whatever legal purpose it may serve
