SWORN AFFIDAVIT OF SOLO PARENT

I, MARITES C. CANDOR, a Filipino citizen of legal age, widow, residing at

Brgy. Mapulo, Taysan, Batangas, after being sworn to in accordance with law,
depose and say that:

1. I am the mother of REGIE C. CANDOR born on April 28, 2007 at

Mapulo Taysan Batangas ;

2. His biological father is the late RUFO A. CANDOR who passed away on
October 20, 2011 at Sinaia, Bauan, Batangas;
 
3. Since the death of my husband up to now, I am the one who is in custody of our
child and supported him both physically and mentally.
 
4. Through this Affidavit, I hereby declare under oath that I’m not
cohabiting with another another partner or co-parent and the one taking
custody and providing support of the above-mentioned child;
 
5. I am executing this Affidavit to attest to the truth and veracity of the
foregoing and for whatever legal purpose the same may serve;
