Good day!

Here's the list of requirements for the request to change the appointed attorney-in-fact.

- Completely filled-out and signed AIF/Loan administrator form (see attached to be accomplished and
signed by the new AIF)
- Valid IDs of new AIF/loan administrator
- Accomplished and signed Customer Request Form (see attached, to be accomplished and signed by
the borrower)
- Request email/letter from the principal borrower to change the AIF (indicate in the subject the type
of request, borrower's name and PN No. or loan account number)

Then send your request at loans-aftersales@bdo.com.ph and may copy me at

gutierrez.markhenry@bdo.com.ph.
