PREFACE
This school year 2020-2021 brought a lot of changes. One of the
major changes in the country is the new set-up in education. For
this, DepEd introduced the LDM Course. This course provides
knowledge to deliver education to learners in this new normal
setting by using Self-Learning Modules.
Teachers reached this far and were able to cope up with the
challenges and demands they are facing in this distance learning
situation because of the learning gained in LDM Course.
This Practicum Portfolio contains MOV’s that we have used in this
school year in delivering our lessons despite of this pandemic. It
contains various artifcats like WHLP, ILMP, Worksheets , LAS,
Ceritificates and even the conversation made with the parents and
teachers

ANNOTATION:
This ILMP serves as a document that will show if the
learner has shown mastery of the lesson if there is a
significant or insignificant progress. This tool can be
used by teachers for the learners who lagged behind
as shown by the result of their assessments.
ANNOTATION:
This Lesson Plan I created a lesson aligned based on the MELCs, where Learning Activities, I make sure to make performance tasks
where they can showcase their creativity.

ANNOTATION:
Learner’s output should be aligned with learning objectives. In making lesson plans, evaluation or assessment must jive to the
MELCs. I also make sure that the learning objectives are being achieved at the end of every activity.

ANNOTATION:
To improve and develop my weaknesses and maintain my strength –I attended seminars and trainings thru online/virtual
and face to face. These trainings and seminars highlighted skills, knowledge, approaches, methods and strategies to become
effeicient and effettive educator or teacher and thourgh this, I have learned a lot on different apps, technologies, strategies and
approaches that are applicable on our new normal set-up.

7.5 Professional Development Goals
ANNOTATION:
For my professional growth and development- I enrolled Masters major in Social Studies which is related to my major as
Socail Studies Teacher at University of Eastern Philippines and also I allow myself to explore and open to any possibilities in
order to develop my speaking skills , in any event or program that totally a big help for me as a teacher to become effective
and efficient speaker.

7.4 Professional reflection and Laerning to improve practice.
LDM Course help us to assess, identify and reflect our strengths and weaknesses being a teacher amid Covid-19
Pandemic. I believed that being a teacher-we need to highlight and overcome our weaknesses for us to improve
and for us to be a better and excellent teacher. LDM Course taught me to establish a learner-centered culture by
using teaching strategies that respond to their linguistic, cultural, socio-economic and religious backgrounds. In
order to give respect and give emphasize the diversity and individual skill of the benefactor of education (student)
– I planned and delivered teaching strategies that are responsive to the special educational needs of learners in
difficult circumstances, including: geographic, isolation; chronic illness; displacement due to armed conflict,
urban resettlement or disasters; child abuse and child labor practices. LDM Course gives assurances and
benefits in terms of Teacher-Learner relationship. Its climax is to give a democratic and harmonious
understanding and connection between the teacher and student by providing timely, accurate and constructive
feedback to improve learner performances.
ANNOTATION:
Quality education amid COVID-19 Pandemic is indispensable especially to the learners of century for
them to be educated and empowered of the content which aligned to the MELCs. Education to the young

5.2 Monitoring and evaluation of learners progress and Acheivement
ANNOTATION:
I monitored learners’ progress by checking and recording their outputs. I immediately give feedbacks and
reminders through personal messages and text messages. I also conducted home visitation for the
learners who lagged and updating their performance and also to remind parents to guide their children
to theirs studies

ANNOTATION:
To have a good and excellent lingkages to my colleagues we created GC for us to be inform
regarding the important matters in our work. And we conducted LAC Sessions and INSET for our
growth and development.

5.4 Achievements to key Stakeholders.
ANNOTATION:
In order for the teachers to inform the learners needs , progress and achievements-we conducted a
quarterly Portfolio Day and Home Visitations. During those days, we discussed everything regarding the
performances of their students and give some feedbacks for their improvement.

6.2 Engagement of Parents and the wider school community in the Educatiive
Process
ANNOTATION:
Since we don’t have enough time to monitor and discuss everything about the topics/lessons- I asked the
parents to help me in monitoring their students and give some assistance in answering their Learning
Activity Sheets.
Thank You
