1 Project Description
PT Manyar Maju Refinery (PTMMR) is constructing a new greenfield Gold Production Facility
located at the Java Integrated Industrial & Ports (JIIPE) site in the regency of Gresik, East
Java, Indonesia. JIIPE is located about 40 km northwest of Surabaya and about 20 km north
of Gresik City. The Scope of Work for package CM002 includes the supply and installation of
the structural, mechanical, piping, electrical and instrumentation.
The project is at EPC phase conducted by PT. Petrosea, Tbk as the main Contractor.

2 Definition and Abbreviation


The following definitions shall apply throughout this Scope of Work:
OWNER : PT MANYAR MAJU REFINERY (PTMMR)
CONTRACTOR : PT. Petrosea, Tbk.(PTP)
SUBCONTRACTOR : The person, Firm, or Company to whom CONTRACTOR’s
Sub-Contract or Sub-Consultant or Purchase Order for
Services or Work is made.
VENDOR : Party which manufacturer or supply the Equipment or Material
to perform the duties specified by CONTRACTOR.
PROJECT : MANYAR MAJU REFINERY (MMR) PROJECT
WORK : Means everything which by the Contractor’s Sub-Contract is
required to be performed by SUB-CONTRACTOR.
EQUIPMENT : Means the machinery, plant, apparatus, materials, articles and
things of all kinds to be supplied by Vendor under the Sub-
Contract or Purchase Order.

3 Scope of Sub-Contractor’s Supply, Work and Service


The subcontractor is requested to provide the rental of scaffolding as well as the labour to erect
and dismantle it afterwards, engineering and calculation for the scaffolding. The subcontractor
is to provide, erect, certify, maintain, and dismantle of scaffolding for the PTMMR Project.

4 Work Included
4.1 Preparation and General Work
a) Provide manpower including equipment, tools, consumables, and PPE.
b) Provide registered/professional/competent engineers to perform design, calculation,
supervise, and inspect.
c) Scaffolding technicians/crew shall be well-trained in erecting, adjusting, dismantling the
system, connecting scaffolding, ensuring that the scaffolding system is installed
correctly and quickly, ensuring sustainability and safety for construction.
d) Provide survey, measurement, marking, and reporting.
e) Mobilization & demobilization of all Subcontractor’s personnel, equipment, tools, etc.
f) Local transport for sub-contractor’s personnel.
g) Compliance with Contractor’s and Client’s Quality & HSE management systems and
procedures.

h) Responsible for all required sub-contractor’s permits and licences to conduct the
Works, which include work permits from Government, Owner, Contractor, etc.
i) Provide all necessary first aid kits for Sub-contractor employees. The clinic, site doctor
and emergency evacuation will be provided by Contractor.
j) Responsible for all relevant sub-contractor’s insurances (i.e. BPJS, etc.).
k) Subcontractor shall responsible and consider site specific conditions (i.e., site terrain,
remote area, limited communication, logistic and transport issues, weather conditions,
etc). Fails to identify this specific site condition shall be Subcontractor’s responsibility.
l) Loading, transport, and unloading of materials from subcontractor warehouse to site
project.
m) Loading, transport, and unloading of materials inside project site area.
n) Supply and installation of temporary supports, barricade, guarding, tagging, and all
other items necessary to ensure safe access and safe working areas are provided
during all stages of the Works on erection.
o) Coordination with Client, Contractor, other subcontractor, and vendors as necessary to
complete the Works.
p) Scaffolding structure is required to be certified safe to use.
q) Erecting, altering, and dismantling of scaffolding must be built in accordance with
OSHA.
r) Interaction with other Subcontractors engaged on the site.
s) Coordinate, co-operate, attend meetings and schedule proposed activities with
Contractor to permit coordination with others.
t) Visible weatherproof sign board, with the completed conformance certificate and "safe
to use" sign to be attached to the erected scaffolding before it can be used.
u) The scaffolding is to be inspected weekly and after inclement weather for corrections
and possible adjustments.

4.2 Scaffolding Requirement
The equipment/material Sub-contractor’s Work shall comply with all relevant requirements
specified but not limited to:
a) Scaffolding installation services for project with various sizes and properties strictly
apply the parameters specified by the BS1139 standard.
b) Scaffolding system made up of galvanized steel pipes that are tightly connected by
accessories such as swing shackles, couplers, clamps, putlogs, boards, ladders, etc.

4.3 Registered/Professional/Competent Engineer


Subcontractor shall include Registered/Professional/Competent Engineer site based to
provide technical support.
Engineer’s role on site will include:
a) Perform design, calculations, supervise, and inspect for the scaffolding erection including
prepare the installation and work method to align with the standard.
b) To initiate, describes the sequence, procedures, and responsible for safe construction and
during use.
c) Assist with construction team and sequencing for construction plan.
d) Provide technical support in matters relating to constructability including assisting in
understanding and interpreting the design.
e) Evaluation monitoring of subcontractors to determine conformance with the Project Plan.

4.4 Site Rooster


a) Staff Roster is on a 6 weeks on / 2 weeks off basis. The personnel will leave site on the
first day of the 2 weeks off and arrive on site on the last day of the 2 weeks off.
b) Non-Staff Roster is on a 9 weeks on / 2 weeks off basis. The personnel will leave site on
the first day of the 2 weeks off and arrive on site on the last day of the 2 weeks off.
c) Site working hours is 10 hours per day (06:00 am until 17:00 pm with 1 hour lunch break),
7 days a week with a Rostered Day Off every 14th day.
d) The key personnel shall have back-to-back, and the swing is managed in such a way to
provide a continuous presence on site.

4.5 Meetings and Reporting


Attending meetings as requested by Contractor, not limited to:
o Coordination meetings with other parties, including with the Client, Contractor,
Vendors and other Sub-contractors.

o Others to support permit, coordination, and audit as necessary.

5 Subcontractor’s Proposal
The subcontractor shall provide at least the following information with their quotation /
proposal:
a) Vendor to submit priced quotation / proposal with 2 types of contracts:
 Installed volume
 Duration contract (per month, 3 months, or etc)
b) Preliminary, vendor to provide the material as following:

e) Subcontractor’s project management and/or engineering management plan.
f) Statement of Compliance or Exceptions, clarifications and/or deviations list to any
referenced documents as part of the complete Request of Quotation (RFQ) package.
g) Priced quotation / proposal.
h) Project organisation chart.
i) List of current projects and workload forecast for the next 1 year.
j) Company financial statement for the last 3 years (preferably audited).
k) List of previous disputes related to provision of Services.
l) Provision of certificate of currency (CoC) for insurance related with the Services.

All deviation from requirements of referenced tender documents, their attachment, codes, and
standard shall be stated in the quotation. In absence of such deviation statement from
subcontractor, Contractor will notice as a full compliance.

5.1 Clarifications, Deviations and Technical Queries


If subcontractor seek any relaxation waiver or exception of any requirement of the requirement
of this scope of work. The subcontractor shall complete a Technical Deviations List PTP-
PROC-FO-G-0526.

In the event of after Purchase Order placement / Sub-contract agreement, the subcontractor
shall submit a Technical Query (TQ). The TQ shall be submitted to the Contractor in the
prescribed format to resolve all technical & quality issues.
The TQ must clearly identify all elements of the proposed relaxation, waiver, or exception in
accordance with the instructions and shall detail any resulting technical, commercial and/or
schedule impact.
