I am Johana O. Bongcarawan, Third year student of Mindanao State University from the College of
Education taking up Bachelor of Secondary Education major in Filipino. Being a student in this school is
such a privilege and opportunity for my academe.

In line with this, I am humbly requesting your office for the Social Welfare Assistantship Program
(SWAP).

I am applying for this program because of certain reasons. First, my mother is a housewife while my
father is a farmer. Second, living far away from them just to study in here made me realized that I also
need to look for some part-time which can support myself and my survival here at MSU.

This, if I would be chosen as part of the program, rest assured that I will be committed to my work and
be a responsible student. Your approval will surely be great and a blessing indeed.
