Job Offer Letter Dr. <name> DMD

Dear Dr. <name>,

It was very nice talking to you over the phone. I wanted to share compensation details
below.

- 33% ADJ Production (Minus dental x-rays, Hygiene in presence of a hygienist)

- $16,000 minimum monthly guarantee (M-F and 2 Saturdays a month)
- $1500 CE annual reimbursement (Two years term)
- Annual reimbursement for malpractice insurance (Two years term)
- Health / Vision / Life insurance
- Free access to internal CE platforms
- $10,000 Sign-on bonus

Lab Fees:
- 33% lab fees paid by the doctors, 77% paid by the company

We believe in establishing long term relationship with our family of doctors. It is important
for me to provide a competitive salary package and support structure for the doctors to be
in a healthy and ethical work environment. I would be more than happy to provide you
with phone numbers of some of our existing doctors who have worked with us for about 3-
5 years, some who have even become partners in their own practices.
