DATE : This is to issue you a memo for having been absent without official leave (AWOL) at work last

As stated in AWOL Policy, pages 33-34 of XX Crew Handbook, an employee is considered AWOL (Absent Without Official
Leave) if

 an employee absents himself from work without officially notifying his/her Manager and obtaining the latter's approval

 if he fails to return to work on the day immediately following the expiration of the period of the employee's approved
leave of absence

 an employee fails to show up for work after one hour from his/her work schedule, unless the employee is able to secure
the approval of the Manager to consider the absence as an Excused Absence (EA) or an Excused Late (EL.).

AWOL shall be subjected to the following schedule of penalties based on AWOL Matrix:

1 - 2 days Written Warning 7 days suspension 15 days suspension 30 days suspension Termination

3 - 5 days 7 days suspension 15 days suspension 30 days suspension Termination

6 - 9 days 15 days suspension 30 days suspension Termination  

11- 15 days 30 days suspension Termination    

16 days and above Termination      

Incidents of AWOL mentioned above falls under the AWOL matrix classification ranging from 1 to 2 (a total of __1______
consecutive days) at 1st offense.

In view of the said offense, the Company hereby gives you a written warning

You are hereby reminded that any further violation of the AWOL Policy shall merit appropriate progressive disciplinary action
provided in the Crew Handbook.
