Annex L: SUPERVISOR FEEDBACK FORM

Intern Performance Evaluation


Supervisors must provide feedback designed to give the intern a constructive assessment
concerning his or her performance during the course of the internship. A supervisor may
provide this feedback orally or in written form. The use of this evaluation form is optional.
Should a supervisor decide to provide the intern with some structured feedback, this form
may be utilized. The feedback provided to the intern in any manner is confidential and
solely for the intern’s benefit. Please take some time to discuss your feedback with
the intern.

Part I
The following scale is provided to assist the supervisor in assigning the most appropriate
rating of the intern’s performance.

1 = Major Improvements Needed/Poor


2 = Some Improvements Needed/Fair
3 = Meets Expectations/Adequate
4 = Often Exceeds Expectations/Good
5 = Consistently Exceeds Expectations/Excellent
