Dear Kate Barrow,

On behalf of Southern Eye Group, I am pleased to issue this offer letter which sets out the proposed terms
for your employment as an Ophthalmic Technician with Southern Eye Group. Compensation is based on
40 hours/week employment with Southern Eye Group.

1. Term: You will provide services for Southern Eye Group and underlying entities; this position is physically located
in Mobile and Baldwin County AL, with others as needed. The Foley AL location will be considered your home location
for travel reimbursement purposes.

2. Base Compensation: Your compensation for the term of employment will be $15.00/hour (payable bi-weekly)
before withholding taxes and voluntary.

3. Benefits: In addition to your base compensation, you will be allowed to participate in the fringe benefit plans
adopted by the practice upon completion of the requirements for eligibility of the respective plan or plans, including, but
not limited to, employer-sponsored health insurance, 401K Safe Harbor Matching Program, and PTO accrual pending
eligibility requirements are met.

On your first day of employment, you will be given additional information about procedures, policies, benefit programs
and more. While we look forward to a mutually beneficial working relationship, it is important to mention that Alabama
is an “at will” employment state. This provides both you and us the right to terminate the employment relationship at any
time and for any reason, with or without cause.
