INTRODUCTION

NARRATIVE REPORT
On the 6th of October 2020, we, the BSN Level II, Block A had an orientation with our
clinical instructor. Discussed from the orientation is a brief history of the community that
we are supposed to visit (General Tinio, Nueva Ecija). Also, a short discussion of the
activities that we will be doing throughout this subject. As part of the activities, we are
tasked to interview five families, input that into a survey form, and consolidate the data
On the 13th of October, through google meet our clinical instructor recheck our
consolidation and discussed those parts that we need to improve. The following day, she
discussed every aspect of the family case analysis that we will be doing, like the FNCP,
In the third week of October, we accomplished our Family Case Analysis and sent it
that day.
Fourth Week: On the 27th day of October 2020, she presented the data of Barangay
Poblacion West and discussed how we could accomplish another task that we’re given to
us: The Barangay profile. After that day, she sent us references as our guide in doing this.
Also, she evaluated parts from the family case analysis that we should have PowerPoint to
present those data. Moreover, in the same week, we we’re able to send the profile analysis
We are now in the fifth week, as we accomplished the Family Case Analysis video
presentation.
The new normal experience of Community Health Nursing is probably different from
the actual. Instead of having home visits in the assigned community or Barangay to be
specific, we are all making it real as possible as we can while we are at our home. Our
clinical instructor gave us data to analyze, make a study, and put it up altogether.

Nursing Students nowadays experienced a big difference before pandemic happen,
from face to face to online class. The students before can communicate and gather data by
talking personally to the families at Barangay Poblacion West. There’s exposure at the said
Barangay, unlike today. We are more on the visualization of what kind of place it looks like
and about the data gathered. We need to admit that we imagine how these people look. The
curiosity is at its highest point. Their houses, the way they dress, and their physical looks
are based on the said diseases they have. Our clinical instructor once said that every
student assigned there could take a group picture in front of Municipal Hall. We also see the
pictures of our Ate’s and Kuya’s that they need to cross the river to take a survey for the
We’ve missed many outdoor experiences in our Community Health Nursing, but we
as Students need to adjust. Yes, there are many great opportunities and the best
experiences we’ve missed, but the knowledge we gain is more important. Amidst this
pandemic, our Clinical Instructors carried out well on how to provide the best knowledge
that we can use in the near future and especially after this pandemic, there’s a chance that
finally, we can see it for real and have a face to face interview with the people in the
community.

General Tinio, officially the Municipality of General Tinio, is a 1st class municipality
in Nueva Ecija, Philippines. According to the 2015 census, it has a population of 47,865
people.
The town is at the Sierra Madre Mountains' foot, adjoining the Fort Magsaysay Army
According to the story passed on from one generation to another, the town got its
name because of miscommunication between the natives and the Spanish colonizers. A
native settler, when asked by a Spanish soldier, "Llama el pueblo?" replied "Papaya"
referring to the particular fruit tree growing abundantly in the place (not understanding
the Spanish language). The name stuck to the Spanish colonizers, and the place from then
Papaya was a Sitio of Barrio Mapisong, which was then a barrio of Gapan (now Gapan
City). Barrio Mapisong became a town in 1851, changing its name to Peñaranda after a
Spanish engineer Jose Maria Peñaranda. Papaya became one of its barangays. Papaya
became a town on January 1, 1921, through the collaborative efforts of Capitan Mamerto
Padolina, who was then the Secretary of the Governor of the Province, Judge Segundo
to General Tinio in honor of General Manuel Tinio, a noble and prominent revolutionary
leader against the Spaniards who hailed from the Nueva Ecija. The act was signed into law
on June 20, 1957, as Republic Act No. 1665. The new name of the town was inaugurated

Barangay Poblacion West was created way back in 1981. It was known as Barangay 7
Poblacion West, General Tinio Papaya, and Santiago Bolisay served as the first Barangay
Captain of the said barangay. Then he was succeeded by kind and generous chairmen such
The Barangay was just a simple community way back then with vegetable planting as
their way of living. Until Primitivo Bolisay became the Barangay Chairman and divided it
into
four Purok or zones. Succession continues, and the barangay became more developed
through Dan Pajarillaga, Benito Bolisay, and the first chairwoman of the barangay, Arlene
Fajardo. The present barangay chairman, Joel Bote (2010-present), made the barangay
livelier. The barangay's name changed and became the Barangay Poblacion West. He also
made another zone in the barangay to make the barangay a 5-zoned community.

Purok 2 is represented by the color violet houses. There are 103 total houses, 98 of
it was surveyed, two (2) of it was unsurveyed, one (1) resthouse and two (2) under
construction. In Purok 2, we also have Brgy. Hall near at Brgy. Center, Gasoline station and

Purok 3 is represented with the color light blue houses. There are 99 total Surveyed
Houses, three (3) unsurveyed houses, zero (0) rest houses (but as per our visit via Google
Map, there are two additional rest houses), two (2) under construction, zero (0)
Bodega/Warehouse and 1 Abandoned House. Also, in this Purok, there are two (2) stores
and one (1) furniture shop that can be found, as well as the river that can be located at the

Purok 4 is presented with green color, and has 63 total number of houses, 55 total
number of surveyed houses, three (3) total number of un-surveyed houses, one (1)
abandoned house, two (2) total number of warehouse or bodega, and two (2) total number
of construction houses. The day care center of the Barangay Poblacion West is also located
at this Purok, we also have here two (2) computer shops, seven (7) stores, a water station, a
canteen, and a resort which is the Gubat Resort located at the south part of this Purok.

Purok 5 is represented by the color blue houses. There are 36 total houses. 30 of
it was surveyed, two (2) under construction, zero (0) abandoned houses, zero (0) rest
houses and three (3) warehouses. In Purok 5, there are two (2) landmarks the Sierra Casas
Hotel, and the Simvy’s Garden and Resort. There’s also two (2) canteens and one (1)
electronic Shop.

Poblacion West’s total number of houses is 382, the students interviewed only 365, and
12 houses remained unsurveyed. The barangay has 2 rest houses, 9 houses under
construction, 2 abandoned houses, and 5 houses that serve as storage houses or bodegas.
There are 513 families residing at the barangay, 365 are nuclear families, and 148 are
extended families. The barangay has a total population of 1862, 918 are male, and 944 are
female. There are 30 children aging 0 – 11 months and 208 at 0 – 71 months of age. The
number of women of reproductive age is 481, married women of reproductive age are 288
and married couples of reproductive ages are 265. The number of out of school youth is 13.

categories, the small family has a 48.54% or 249 families which has family members of 1 to
3, the medium family has 46.59% or 239 families, which has a total number of family

of more than seven family members.

The length of residency of the 513 families are categorized into two; one is the
permanent or the families that are residents of more than six months and has a 97.66% or
501 of the total number. The other category is temporary or transient; these are the
families living in the barangay, and only renting the house where they live in it has 2.34% or
12 of the total number.

a number of 7.02% or 36. The family with a preschooler or the family with firstborn

their firstborn ages 7 to 13 with a total population of 16.96% or 87. With their firstborn
aging 14 to 21, the family with a teenager has a total number of 18.52% or 95.

disadvantages of having low income means that the family cannot reach their family felt
needs. Also having a low income can be source of discrimination, it is because a person can
judge their capabilities by having low income. While the lowest percentage is 40,000-

Besides they are not prone in criticism and judgements. The disadvantages of having high
monthly income, the family lacks bonding time and affection to each other because they can

Electricity has many uses in our day to day life. It is used for lighting rooms, working
fans and domestic appliances like using electric stoves, A/C and more. All of these provide
comfort to people. Moreover, power/ lighting source is important factor to consider in a
family data since the use of kerosene, LPG/ lamp and others can cause harm to the family.
Breathing large quantities of kerosene vapour may cause non-specific signs such as
dizziness, headache and vomiting. Repeated skin exposure may result in dermatitis
(eczema) (WHO, 2020). On the other hand, LPG vapours can run for long distances along
the ground and can collect in drains or basements. When the gas meets a source of ignition
it can burn or explode. Cylinders can explode if involved in a fire. LPG can cause cold burns
to the skin and it can act as an asphyxiant at high concentrations (HAS, 2015).
In contrast, based on our gathered data, the majority of the population, uses
electricity as their power source with a percentage of 99.18 % which is a good indication
and it only implies that the people in the community have a common source of power and
lighting.

Public transportation can convey many more people in much less space than
individual automobiles, which helps to keep traffic congestion lower, which in turn reduces
air pollution from idling vehicles, and helps riders avoid the stress that comes from daily

Based on the gathered data, the majority of the population in Poblacion West,
General Tinio Nueva Ecija prefer to use bus and tricycles as their means of transportation
with a percentage of 34.14, following the use of single motorcycle with a percentage of
14.34%.
These indicate that the majority of the family prefers these means of transportation
since the public bus is cheaper and tricycle is the fastest way to arrive at their designated
destination. Furthermore, Brgy. Poblacion West owned a single motorcycle and prefer to
use it for them to save money.

Based on the gathered data on the married woman of reproductive age, the highest
with the total number of 35 are ages 35 to 36 years old. The least with the total number of 0
are ages 15 to 16 years old.
It implies that proper education with regards to the importance of reproductive
health must be reinforced in order to increase the awareness of the proper ways on how to
take care of their own health.

Family planning acceptors wherein they are using various type of family planning
methods like pills, injectable, bilateral tubal ligation, Lam, and others.
It only implies that there will be a high possibility of an increased in the population
in the following years due to the high percentage of married woman of reproductive age
that are non-acceptors of family planning.

Based on the gathered data on the tetanus toxoid, on-going has the highest
percentage with 10 (83.33%) followed by the no immunization and fully immunized
pregnant women with 1 (8.33%)
As a health care provider, it is good to know that majority of the pregnant women
are engaged with the said immunization because it implies that they have the knowledge
and capabilities to protect themselves as well as their baby in their womb.

This implies that majority of the population in the community are not aware of the

organization itself and the benefits and support that they can get. There may also be a lack
of promotion and lack of recruitment strategy from the organizations.

Based on the data that we have collected, haranista and senior citizens have the
same highest percentage of these organizations, gathering 26 or 31.35% of the population.
It implies that these organizations help the people in terms of exemption on income tax,
medical services, financial assistance, discounted goods, Philhealth membership, and
express lanes.
Next on the list is the 4P's organization, with 20 people or 24.10% of the population.
This implies that some of the people are financially unstable. The member of this
organization receives benefits, which are monthly allowance and seminars from the
government.

The third is the sb-toda organization, with 6 or 7.23% of the population. This implies
that people see this as helpful for those who work as tricycle drivers. The benefits are they
can gain more profits in their area.
Fourth is katekista organization with 2 or 2.41 of the population. This is helpful to
the people as they will learn through church guidance and be morally acceptable in society.
Last on the list is the Toda, Oro Fraternity, Guardian, and Kababaihan organization, which
have the same number of 1 or 1.21% of the total population. This implies that people also
receive benefits from a gender-based group to maintain peace, create works, and calamity
response.

Based on the data that we have gathered, 279 (76.44%) houses are not aware of the
organization that exists in the community. It means that many of them lack knowledge
about the existing organizations in their community; this may result from a lack of
information dissemination in their community. On the other hand, 86 (23.56%) houses are
aware of the existing organization, which means some knowledge of the current
organization in their community even if they are busy with their daily lives.
Residents of the community must be informed about the existing organizations in
their community to gain knowledge about the projects and programs they will participate
in. Those who have experience in the current organizations must also encourage those who
lacked so that they will be oriented and become aware.

The graph and table show the result of the list of leaders whom can lead the community
stated by the people of Poblacion West. The ranking is based on the number of times those
names are mentioned by the interviewees.
Out of 1, 862 population in Poblacion West, 688 of them mentioned the potential
leaders listed above. The top five leaders among the nine are, Joel Bote who has 257 votes,
Sally Arevalo who has 137 votes, Feliman Manabat who has 117 votes, Narciso Maducdoc
who has 86 votes, and Roel Bote who has 38 votes.
This implies that there are outstanding citizens in the community that has shown
potential to be a leader, and is visible to everyone. With that, it will be easy for the
community to choose a great leader who will help them rise or improve through a good

responded with a No. For the adequate exercise, 239 out of the total surveyed population
responded with a Yes while 126 said No. Lastly, for relaxation and stress management, 345
out of the total surveyed population responded with Yes while 20 responded with No.
Based on the gathered data from Poblacion West, a total of 89.9 % have an adequate
rest and sleep while the remaining doesn’t and that is 10.1% in the total population is at
risk. A total of 65.5% have an adequate exercise while a total number of 34.5% doesn’t have
an adequate exercise. For adequate relaxation and stress management, 94.5% of the total
population have an adequate relaxation and stress management while the remaining 5.5%
are at risk for health problems due to lack of relaxation and stress management.
Among the three categories, the category with the highest adequacy is relaxation and
stress management with the total of 345 people out of the 356 total population of Barangay
Poblacion West. On the other hand, the category with the highest inadequacy is exercise
with 126 people responding with a no.
Therefore, we can conclude that the Barangay Poblacion West should promote more
physical activities among their Barangay by conducting events that promotes physical
activities such as Zumba or sports activities among the people.
