PRE-ENROLLMENT HEALTH ASSESSMENT (PEHA) 2023

Whether you’re a new student (UPCAT freshmen, graduate/post-graduate, VAAS*, certificate,


special program), a returning student (readmission from LOA/AWOL of more than one semester),
or a transferee from another UP campus or university—to enroll at UP Diliman, you’ll need to
complete your Pre-enrollment Health Assessment (PEHA). Just follow these steps:

1. Download and digitally accomplish all PEHA Forms then print them (paper size should be either
short or A4).

Can’t fill out the forms digitally?


Don’t worry, you could go the handwritten route. However, be sure to use BLACK or
BLUE INK and WRITE LEGIBLY. Our doctors at the UP Health Service (UPHS) will be
going through all forms manually, so legibility is definitely important.

2. If you have any comorbidity or chronic illness, or if you’re immunocompromised, visit your
attending physician and obtain a medical certificate allowing you to attend classes and/or
stating your restrictions.

3. Done with all the required paperwork? It’s finally time to have your diagnostic tests.
You have two options here:

Option A: Have the following done at your preferred reputable healthcare facility:

a. Chest X-ray (PA view): should be LESS than six months old
b. Complete Blood Count: should be LESS than a week old if PE will be done at UPHS
c. Urinalysis: should be LESS than a week old if PE will be done at UPHS

Option B: Get your diagnostic tests done at UPHS

It’s fine if you have recently undergone some of the tests listed above—you can still have
the remaining ones done at UPHS. But remember, results will be ready for pick-up after
one (1) to three (3) business days.

Note: You may also have your dental assessment at UPHS during your visit for diagnostic
tests, no appointment needed

REMINDERS:
i. You may only proceed with the next step ONCE your diagnostic test results and PEHA forms
are COMPLETE.

ii. *Students admitted through the VAAS program (athletes) must follow the same instructions
AND must present an ELECTROCARDIOGRAM (ECG) result in addition to the required
diagnostic tests. If your ECG findings are abnormal, seek clearance from a Cardiologist
before proceeding to Step 4.
4. Are you ready for your physical examination and mental health assessment?
There are also two ways of doing this:

Option A: Have your physical examination at a healthcare facility other than UPHS

A.1. Provide your doctor with the results of your diagnostic tests, and have him/her
accomplish PE Form 2C. Afterwards, consult your dentist and ask him/her
to fill out the Dental Chart and Recommendation Sheet.

A.2. Once you’re done, Book an appointment for in-person consultation at


UPHS. This step is for validating your physical exam as well as for
completing your mental health assessment.

Option B: Have your physical examination and mental health assessment at UPHS

B.1. Book an appointment for in-person consultation at UPHS.

B.2. Gather and arrange ALL PEHA requirements (see list below) then proceed
to UPHS OPD for your scheduled in-person consultation.

✓ PEHA Forms
✓ Results of: Chest X-ray (Radiographic/Radiologic Findings), CBC, Urinalysis
✓ Medical Certificate (comorbidity/chronic illness), if applicable

Are walk-ins allowed?


Only those with complete physical and dental assessments will be allowed to
WALK-IN (no appointment) Monday to Friday, 9:00am to 3:00pm, for validation
of physical examination and for mental health assessment.

Upon completing the Pre-enrollment Health Assessment, you will be given Medical Certificate for
Admission. With it, you can proceed with your enrollment at UP Diliman.
