Thank you for participating in Wolfpack Information Risk’s 2023 SA Information Security
Thermometer study, the biggest of its kind in South Africa. This project was first conducted in
2011. The critically acclaimed results yielded valuable insight that empowered information
security stakeholders to compare themselves to organisations of a similar demographic to
make more informed decisions. 

More than a decade has passed since then, with significant changes observed in the
industry. There is no better time than now to conduct this study again, to give the industry a
perspective of movement over time, and to unpack the current state of information security
across ten key domains.

A study of this nature can only enjoy success through the quality of the input. As a South
African information security stakeholder, your voice and perspective is extremely valuable.
As such, we urge you to answer each question to your best knowledge, as
comprehensively as possible. 

Your input will be confidential, treated with integrity, and not be traced back to you. All
personal information will be protected in compliance with POPIA.

I hereby agree to participate in this study (“the Study”)  and/or to provide my opinion regarding the
subject of the Study, including, if applicable, the Products.  HDS Global (“the Service Provider”) is
mandated by Wolfpack (“the Sponsor”).

Before agreeing to participate in the Study, I understand that it is important that I read, understand
and agree with the following:

1. PERSONAL DATA

In terms of Section 18 of the Protection of Personal Information Act No. 4 of 2013 (“POPIA”), I
hereby expressly consent to allowing my personal data (such as first name, last name, email
address, region) to be collected and processed by the Service Provider in South Africa.

The Sponsor will be the Responsible Party and my personal data will be collected directly from me.

My supply of my personal data is voluntary and should I elect not to provide my personal data, then
in consequence thereof, I will not be able to participate in the Study.

I consent to allowing the Sponsor to receive and process the Study data related to me, excluding
my first name last name and mail address.

I understand and consent that my personal data will be used for improving knowledge of
consumers’ needs and practices, and development of products and services to meet these needs
and practices.

My personal data will not be used for any other purpose without my prior written consent.  It will be
stored by the Service Provider/Sponsor for 10 years starting from the end of the Study (though this

period may be extended by the implementation of a legal provision).

In accordance with the Law, I have the right to access, the right to rectify, and the right of erasure
