1.0 OBJECTIVE

The objective of this process is to establish requirement, set guidelines and standard
procedures of quality assurance and quality control activities to ensure compliance with
all applicable statutory, regulatory, customer and other specified requirements.

2.0 SCOPE

This process covers quality assurance and quality control activities applicable to key
aspects of the business process from design, bid, construction and project turn-over.

3.0 DEFINITION OF TERMS

Quality Assurance A part of the Quality Management that focused on providing confidence
that quality requirements will be fulfilled and is a way of preventing
mistakes, defects and problems. Quality Assurance is process oriented
and focuses on defect prevention at the early stage.
Quality Control Is a process by which various parties review the quality of the factors
involved in the delivery of the product which focuses on defect
identification and correction.
For Construction Refers to the plans, drawings or details issued by the Designer of
Drawings (FCD) Record duly approved by the client or its authorized representative
intended for construction works implementation.
Shop Drawings / Refers to a drawing or set of drawings produced by the general
Fabrication Drawings contractor, supplier, manufacturer, subcontractor, or fabricator. Shop
drawings are typically required for prefabricated components. All shop
drawings should be based on the latest For Construction Drawings
(FCD)
Detailed Engineering These are drawings that further elaborates each aspect of the
Drawings project/product by complete description through solid modeling,
specific drawings as well as technical specifications.
Customer Refers to the Owner of the project or its duly authorized
representative. A Customer may be a person or organization that avails
the goods or services.

Operations Section Refers to the section of the organization in a project implementing the
construction work implementation.
QA/QC Quality Assurance / Quality Control
QA/QC Section The implementing group which is in charge of the enforcement of the
activities and procedures of the Quality Assurance / Quality Control
Plan.
Quality Plan This is a systematic and highly structured plan for assuring that all
requirements of the contract documents are accomplished in
conformance with client's specific set of contract approved drawings,
documents, regulations and/or specifications. This includes designing
the specific guidelines to be utilized to fit the scope of work, choosing
qualified personnel for task to be performed, and issuing instructions
to control, inspect and oversee work completion in terms of quality.
Inspection and Test A document detailing a systematic approach to testing or inspecting a
Plan system or product and set out critical control points or 'hold points' at
various stages within a process to make sure that conformance to
standards are progressing as they should be.
Project QA/QC Head Technical Person In-charge of the Project QA/QC Section that maybe
an Engineer or an Architect responsible for the effective
implementation of the QA/QC activities of the project
Project QA/QC Staff A technical personnel that maybe an Engineer, Architect or any
personnel in a particular field of specialization qualified to support
implementation of the QA/QC activities of the project under the
supervision of the Project QA/QC Head.
Approving Entity (AE) An individual or a group who have the authority of officially agreeing
to something or accepting something as satisfactory. This may be
Customer’s or Internally assigned Technical Designer/s and
Consultant/s competent to conduct review and approval of technical
submittals, reports, structure, and/or products.
Responsible Entity Personnel, Department or Company who is responsible and
(RE) accountable for a nonconformity identified.
Issuing Entity (IE) Personnel, Department or Company who has the first-hand knowledge
of the nonconformity identified. The personnel or the Department who
raise and issue the Corrective Action Report
Approval The action of officially agreeing to something or accepting something
as satisfactory to the requirement or expectations.
Nonconformity A condition of “work in place" or "work in progress" that lacks
agreement or conformity to established requirements, and which

cannot be corrected in the normal course of construction activities.


Non-Conformance is reserved for those conditions which will require
major rework efforts, removal and/or replacement of materials or
product.
Punchlist Workmanship deficiency that can be repaired in a manner that will not
require large amount of resources and time. Can be closed by “Making
Good” repairs.
Technical Complete set of documents and references, including drawings and
Specification specifications, which contains the full and total agreement, legally
entered into, between the Owner and the Construction Contractor to
provide a specified product in return for specified remuneration.
Third Party An independent organization, person, or company not affiliated with
Testing/Inspection the producer nor the user of the item that tests/inspects products,
Service Provider materials, software, etc. according to agreed requirements.
Request for Used to clarify details available in For Construction Drawings but vague
Information or to request resolution of conflicts between issued Drawing,
Specification, terms of reference and interfacing work of different
trades.
Calibration The comparison of measurement values delivered by a device under
test with those of a standard of known accuracy
Subcontractor / A person or company engaged by a General Contractor to supply
Supplier materials/equipment or perform a specific services that is part of the
overall project and is normally paid for the supply of
materials/equipment or services provided to the project by the General
Contractor.
General Contractor The contracting company who undertakes a general contract which
includes materials or labor to produce products or perform services.
Consultant A person or company who provides technical expertise or management
services.
Audit Team Leader The SBU QA/QC Manager or his/her authorized representative who is
responsible for the planning and implementation of Project Quality
Audits.
Audit Plan Specific guideline to be followed when conducting an audit. The plan
includes information such as the date, time, audit scope, audit team
and auditee of the audit to be conducted.
Audit Checklist Detailed list of checkpoints covering the scope of the audit.

Defects Liability A period following practical completion (usually one year) during which

and effectiveness of set standards, procedures and identifying impediments
that affect their implementation.

5.6.3 Resolving disputes and perform arbitration between parties regarding quality
related issues.

5.11.5 The Project Manager shall administratively and functionally report to the SBU
Head.

6.0 PROCEDURE

6.1 Preparation and Approval of Quality Plan

6.1.1 The Project QA/QC Head shall prepare a Quality Assurance / Quality Control
Plan prior to work implementation.

6.1.2 The Quality Plan shall include not limited to the following:

1) Objective - This refers to specific desired result or outcome that the


Quality Assurance / Quality Control System aim to achieve.

2) Scope - This defines the extent of application of the Quality Assurance


/ Quality Control Activities.

3) Definition of Terms - Specific description of terms used in the Quality


Plan.

4) Reference Documents - Refers to the set of document, procedures,


actions and strategies related to the particular Quality Assurance /
Quality Control activity.

5) Table of Organization - Refers to the table of personnel, position and


their relation to the entire organization.

6) Responsibilities and Authorities

a) Roles and Responsibilities - This defines the function and


accountabilities of each Quality Assurance / Quality Control
personnel.

b) Authority - This defines the power to influence or command of


each Quality Assurance / Quality Control personnel.

7) Procedures

a) Quality Assurance Activities - These are actions performed to


ensure that each required material or service is conforming to
the Contract Provisions prior to its utilization.

b) Quality Control Activities - These are actions performed to ensure


that each material or service is conforming to the Contract
Provisions during and after utilization.

c) Documentation Process - This defines the process of record


generation, control and filing for the entire project duration.

8) Appendix A: List of Third Party Testing or Inspection Service Providers


- This is a directory of Third Party Testing and Inspection Service
Provider that may be commissioned for the project. Accreditation of
the Third Party Testing and Inspection Service Provider shall be in
accordance with the Supplier/Subcontractor Accreditation and
Performance Monitoring Procedure.

9) Appendix B: Standard Forms - This is a compilation of documents to


be used as standard forms to be used as evidence of Quality Control
records.

6.1.3 The draft Quality Assurance / Quality Control Plan shall be reviewed by the
SBU QA/QC Manager in collaboration with the Project Manager and SBU
Head, and approved by the Corporate QA/QC Head.

6.1.4 The approved Quality Assurance / Quality Control Plan shall be submitted
to the Client for approval and/or reference as required.

6.1.5 The Project QA/QC Head shall ensure that all reference documents required
or stated in the Quality Assurance / Quality Control Plan are available in
the project.

6.1.6 The Project QA/QC Head shall disseminate the approved Quality Plan to
the following as deemed necessary:

1) Project Manager
2) Project-in-Charge
3) Head Office Engineer
4) Project Safety Officer / Project Environmental Officer

6.1.7 The Project QA/QC Head may adjust/amend the Quality Plan or provisions
thereof with the approval of the Corporate QA/QC Head, as necessary at
any stage of the project in accordance with this process.

6.2 Preparation and Approval of Inspection and Test Plan

6.2.1 The Project QA/QC Head shall prepare an Inspection and Test Plan prior to
the work implementation.

6.2.2 The Inspection and Test Plan shall include not limited to the following
information:

1) Work Activity - This identifies the specific work or services intended to


be implemented.

2) Control Document - These are set of documents that contain reference


standards to be conformed with.

3) Control Item - This identifies a particular material or stage of the


process which is subject to inspection to ensure conformance.

4) Procedure Required - This identifies the particular test or inspection


required to be implemented to verify conformance to the reference
standards.

5) Instruments to be used - This describes the measuring tools and


equipment to be used to conduct the test and inspection required.

6) Nonconformance - This describes the action to be taken to meet the


acceptable level of quality required if nonconformity is identified.

7) Frequency of Test Required - This identifies the lot size and frequency
of testing or location/s of inspection required.

8) Reference of Test Method - This refers to the standard set of


procedures in conducting testing and inspection.

9) Hold Points - These are construction stages which need supplementary


inspection or verification. These are activities or requirements to be
complied prior with the commencement of succeeding works.

10) Responsible Parties – This defines the responsibility and authority of


each following entity in the conduct of testing and inspection:

a) Subcontractor / Supplier
b) DMCI
c) Consultant
d) Client

6.2.3 The proposed Inspection Test Plan shall be reviewed by the SBU QA/QC
Manager and approved by the Corporate QA/QC Head.

6.2.4 The approved Inspection and Test Plan shall be submitted to the Client for
approval as necessary.

6.2.5 The Project QA/QC Head shall ensure that all reference documents required
or stated in the Inspection and Test Plan are available in the project

6.2.6 The Project QA/QC Head shall disseminate the approved Inspection and
Test Plan to the following as necessary:

1) Project Manager
2) Project in-Charge
3) Head Office Engineer
4) Project Safety Officer / Project Environmental Officer

6.2.7 The Project QA/QC Head may adjust/amend the Inspection and Test Plan
or provisions thereof, as necessary at any stage of the project in
accordance with this process.

6.3 Review and Approval of Shop Drawings / Fabrication Drawings

The sequence of activities is summarized in the Flow Chart at Appendix 1. The


following supplementary information shall also apply:

6.3.1 Project-in-Charge/Subcontractor/Supplier shall submit shop drawings /


fabrication drawings to Head Office Engineer for review in accordance with
the following relevant documents:

1) Project Technical Specifications


2) Latest For Construction Drawings (FCD)
3) Official Instructions from the Client or Authorized Representative

The proposed Shop Drawing / Fabrication Drawing shall:

1) Detail how the For Construction Drawings (FCD) will be implemented


2) Reflect approved materials and equipment
3) Reflect the scope, limit and exclusions
4) Address interface issues with other trades
5) Based on as-built measurements

Operation/Subcontractor/Supplier shall explicitly inform the Head Office
Engineer and other relevant parties in writing during the submission of the
proposed Shop/Fabrication Drawing for review.

6.3.2 Upon receipt of the proposed Shop Drawing / Fabrication Drawing, the Head
Office Engineer together with the Office Engineer shall:

1) Collate additional information not present in the submission and request


further clarifications.

2) Prepare a Preliminary Drawing Review based on the information

6.3.3 After gathering all the information, the Head Office Engineer shall determine

3) Rejected. Resubmission is not required.

6.3.4 In case of nonconformity, the Head Office Engineer shall mark-up/comment

6.3.5 Upon conformity with all the requirements, the Head Office Engineer shall

6.4.5 The Project QA/QC Head together with the Project QA/QC Staff shall:

1) Collate additional information not present in the submission and request


further clarifications.

2) Consolidate information submitted and coordinate initial technical review


with following relevant parties as necessary:

a) Project-in-Charge for construction related matters


b) Project Safety Officer for OH&S matters
c) Project Environmental Officer for environmental matters
d) Project Manager
e) Designer / Technical Consultant

3) Coordinate the conduct of Preliminary Test or Factory Acceptance Test


(FAT) based on the Approved Inspection and Test Plan. The result of the
Preliminary Test or Factory Acceptance Test shall be included in the
Technical Evaluation of the product or equipment.

4) Prepare a Technical Evaluation of the product or equipment based on


the submitted information and the Preliminary Test conducted. The
Technical Evaluation shall compare and verify the conformity of the
proposed material to the relevant references stated in 6.4.1.

6.4.6 The Project QA/QC Head shall submit the Technical Evaluation to the Head
Office Engineer. The Project QA/QC Head shall state in the Technical
Evaluation if the proposed material in terms of quality is:

1) Recommended for submission to the Approving Entity (AE)


2) To be resubmitted after conformity with the technical nonconformity
identified.
3) Rejected. Resubmission is not required.

In the case of technical nonconformity, the Project QA/QC Head shall


comment on the Technical Evaluation the nonconformity identified and
return the commented submittal to the Head Office Engineer.

6.4.7 In the case of technical conformity, the Head Office Engineer shall conduct
a commercial evaluation of the proposed material / equipment in accordance
with the Procurement Control Procedure.

6.4.8 Upon conformity with both the Technical and Commercial requirements,
the Head Office Engineer shall recommend submission of the Material /
Equipment Submittal to the Approving Entity (AE) for final approval through
the Project QA/QC Head.

6.4.9 The Approving Entity shall determine and indicate in the Submittal Form if
the proposed material / equipment is:

1) Approved
2) Approved with Comments
3) Revise and Resubmit
4) Disapproved

6.4.10 A copy of the commented Material / Equipment Submittal shall be provided

parties such as Owner Direct Subcontractors/Suppliers, they shall be

6.7.11 Delivered equipment shall be located on suitable area for storage designated
areas by Project in Charge. It shall be protected from any damage or

Warehousing Control Procedure.

6.7.12 Nonconforming equipment shall be tag using the Disposition Tag to prevent
its unintended use. The disposition of nonconforming materials shall be in
accordance with the Control of Nonconforming Outputs Process.

6.7.13 When equipment is delivered on behalf of the Client/Owner or other third


parties such as Owner Direct Subcontractors/Suppliers, they shall be

6.8 Inspection of Work Implementation (In Process)

6.8.2 Upon receipt of the Inspection/Work Request, the Project QA/QC Staff under
the supervision of the Project QA/QC Head shall:

1) Coordinate the conduct of preliminary inspection based on the approved


Project Inspection and Test Plan (ITP) to verify conformity with relevant
documents stated in 6.8.1

2) Coordinate the conduct of sampling and testing based on the approved


Project Inspection and Test Plan (ITP) to verify conformity with relevant
documents stated in 6.8.1

3) Request further clarifications if needed.

4) Check conformity of work implementation based on the approved


construction work implementation methodology.

5) Record and issue the identified workmanship deficiencies / punchlists


using the Key Plan or Punchlist Form. The Punchlist Form or marked Key
Plan shall be issued to Operation/Subcontractor immediately after
inspection.

6) Recommend the issuance of Corrective Action Report (CAR) for


nonconformities as necessary. Issuance of CAR shall be in accordance
with the Corrective Action Process.

6.8.3 Upon receipt of the Punchlist Form or commented Key Plan, the
Operation/Subcontractor shall implement the necessary action to comply
with the issued punchlist/s prior to the request of a follow-up inspection.

6.8.4 The Operation/Subcontractor shall inform and request the Project QA/QC
Staff to conduct a follow-up inspection to verify conformity to the issued
punchlist/s.

6.8.5 The Project QA/QC Staff together with Operation/Subcontractor shall


conduct a follow-up inspection to verify conformity to the issued punchlist/s.

If found to be still deficient, the process described in 6.8.3 will be repeated


until conformity.

6.8.6 If all (or relevant as necessary) issued punchlist/s are conformed as verified
during the follow-up inspection/s, the Project QA/QC Staff shall inform the
Project QA/QC Head and recommend the final inspection to the Approving
Entity (AE).

6.8.7 The Approving Entity shall conduct inspection together with Project QA/QC
Staff and Operations and shall determine if the work implementation is to
be:

1) Approved. Work implementation or succeeding works can commence.


2) For Re-Inspection after conformity with deficiencies identified.

6.8.8 Approving Entity (AE) shall indicate the deficiencies identified in the Punchlist
Form or in the Key Plan attached and issue to the Operation/Subcontractor
for correction and closeout. Operation/Subcontractor should request conduct
of a re-inspection once all deficiencies identified are corrected/rectified.

6.8.9 The requested work or succeeding works shall commence once the

6.8.10 If the requested date of Inspection / Work Request expires, Operations to


re-submit new Inspection/Work Request as necessary to reflect the new
schedule of inspection, as required by the Approving Entity (AE).

6.8.11 If deemed necessary, the Project QA/QC Head shall have the authority to
stop work implementation if nonconformity to the approved plans/method
statement and specification was observed.

6.9 Calibration of Measuring Tools and Equipment

The sequence of activities is summarized in the Flow Chart at Appendix 7. The


following supplementary information shall also apply:

6.9.1 The Project QA/QC Staff under the supervision of the Project QA/QC Head
shall inspect at specified intervals or prior to use, the calibration status of all
critical measuring equipment utilized in the work implementation.

6.9.2 All measuring tools and equipment shall be calibrated against measurement
standards traceable to international or national measurement standards.
When no such standard exists, the basis or method used for calibration or
verification shall be retained as documented information.

6.9.3 If calibration is required, the Project QA/QC Head shall initiate the request
for calibration of measuring equipment. Procurement Process shall be
followed in the conduct of request for calibration services.

6.9.4 The Project QA/QC Head shall create and maintain a Tools and Equipment
Calibration Monitoring Log which includes the following information:

1) Name and description of measuring tools & equipment


2) Measurement standard used
3) Date of last calibration
4) Calibration Service Provider
5) Name of User/Custodian
6) Date of Expiration
7) Working status of tools and equipment

6.9.5 All calibration records shall be maintained and recorded in the Tools and
Equipment Calibration Monitoring Log.

6.9.6 The User/Custodian of the measuring equipment shall ensure that calibration
stickers, serial numbers and other forms of markings which show the
calibration status of the equipment shall not be tampered, transferred or
damaged during its usage.

6.9.7 The User/Custodian shall ensure that the measuring equipment shall be
safeguarded from adjustments, damage or deterioration that would
invalidate the calibration status and subsequent measurement results. The

User/Custodian shall submit a status or incident report to the Project QA/QC


Head in any event of:

1) Accidental damage
2) Erratic or inconsistent results
3) Recognizable error
4) New software update
5) Failed result during verification

6.9.8 The Project QA/QC Head shall request the conduct of calibration before the
date of next calibration when status of equipment is in doubt due to the
reason stated in 6.9.7.

6.9.9 When the measuring equipment during the course of work implementation
is found to be unfit for its intended purpose, the Project QA/QC Head shall
determine if the validity of previous measurement results has been adversely
affected and take appropriate action/s on the process, product or services
that these measurements have been utilized as necessary.

6.9.10 No un-calibrated measuring tools and equipment shall be utilized in the work
implementation unless otherwise specified and agreed by concerned parties.

6.9.11 Calibration of Survey Equipment shall be in accordance with the Survey


Control Procedure.

6.9.12 Calibration of testing and measuring equipment of RMC shall be in


accordance with Equipment Commissioning Procedure and Control of
Inspection and Test Equipment of the RMC Manual.

6.9.13 Calibration of testing and measuring equipment of EMD shall be in


accordance with Equipment Commissioning Procedure of the EMD Manual.

6.9.14 The Third Party Testing Service Provider shall be responsible for calibration
of all their measuring and testing equipment. Project QA/QC Head shall
require and secure copies of Calibration Certificates prior to engagement of
testing or inspection services and succeeding engagement thereafter.

6.10 Inspection of Final Products and Services (Turn Over)

This procedure details the Quality Control activities supplementing the Project Turn
Over Process.

The sequence of activities is summarized in the Flow Chart at Appendix 8. The


following supplementary information shall also apply:

6.10.1 Operation/Subcontractor to submit an Inspection Work/Request to the


Project QA/QC Head at least twenty-four (24) hours prior to date of
inspection or work implementation. The Work Request should contain
information not limited to the following:

1) Description of product and services


2) Scope and limit
3) Location
4) Proposed date of inspection

The following supplemental documents should be accompanied during the


submission of the Inspection Work Request:

1) Key Plan showing scope and limit of Turn Over


2) Balance of Work List
3) Handover Acceptance Form
4) Testing & Commissioning Data if applicable
5) Operation and Maintenance Training & Familiarization Certificate if
required.

6.10.2 Upon receipt of the Inspection/Work Request, the Project QA/QC Staff under
the supervision of the Project QA/QC Head shall:

1) Coordinate the conduct of Preliminary Inspection.

2) Collate and consolidate additional information required not present in


the Inspection/Work Request submitted.

3) Request further clarifications if needed.

4) Record and issue the identified workmanship deficiencies / punchlists


using the Punchlist Form or Key Plan. The Punchlist Form or Key Plan
shall be issued to Operation/Subcontractor immediately after inspection.

5) Recommend the issuance of Corrective Action Report (CAR) if necessary.


Issuance of CAR shall be in accordance with the Corrective Action
Process.

6.10.3 Upon receipt of the Punchlist Form or Key Plan, the Operation/Subcontractor
shall implement the necessary action to comply with the issued punchlist
prior to the request of a follow-up inspection.

6.10.4 The Operation/Subcontractor shall inform and request the Project QA/QC
Staff to conduct a follow-up inspection to verify conformity to the issued
punchlist/s.

6.10.5 The Project QA/QC Staff together with Operation/Subcontractor shall


conduct a follow-up inspection to verify conformity to the issued punchlist.
If found to be deficient, the process described in 6.10.3 will be repeated until
conformity.

6.10.6 If all (or substantial as required) issued punchlist are conformed as verified
during the follow-up Inspection/s, the Project QA/QC Staff shall inform the
Project QA/QC Head and recommend the Final Inspection to the Approving
Entity.

6.10.7 The Approving Entity shall conduct inspection and shall determine if the
product or service is to be:

1) Accepted. Approving Entity (AE) to sign and accept the Handover Form.

2) For Re-Inspection after conformity with deficiencies identified.

6.10.8 In the case of re-inspection, the Approving Entity (AE) shall issue a Punchlist
to the Operation/Subcontractor for conformance. Operation/Subcontractor
to request conduct of a re-inspection once all deficiencies identified are
rectified.

6.10.9 After acceptance of product or service, documentary requirement for Turn-


Over shall be submitted to the Approving Entity(AE) in accordance with the
Project Turn Over Process.

6.11 Review of Turn-over Documents

The sequence of activities is summarized in the Flow Chart at Appendix 9. The


following supplementary information shall also apply:

6.11.1 Project QA/QC Head together with the QA/QC Staff shall review the following
Turn-over documents, as required:

1) Signed-off Request for Inspection - This includes the compilation of


issued Work Inspection Request duly signed by the Approving Entity.

2) Certification from Project Engineer on the rectification / correction works


supported with photographs. - This includes the compilation of closed
Corrective Action Reports and/or Punchlist Form issued during the entire
duration of the Project.

3) Contract, NTP, and Narrative Scope of Work including Variation Order /


Supplemental Contract (if any) - This includes issued and recorded
Contractual Documents related to the Project.

4) Approved Program of Work (POW) - This includes the approved project


schedule, manpower and equipment loading.

Project.

7) Original Plans and As-built Drawings - This includes the compilation of


Issued For Construction Drawings, Conceptual Engineering Drawings,
Detailed Engineering Drawings (for Design and Build Projects) and As-
Built Drawings. As-built Drawings are the revised set of drawing
submitted by the Contractor upon completion of a project or a particular
job. They reflect all changes made in the specifications and working
drawings during the construction process, and show the exact
dimensions, geometry, and location of all elements of the work
completed under the contract.

8) Updated Technical Specification - Updated Technical Specifications due


to changes/amendments during construction.

9) Design Calculations – Documented evidence of design outputs


determined or ascertained through mathematical methods.

10) Performance / Warranty Securities - A copy of the Performance /


Warranty Security shall be submitted to the Employer which shall cover
the Defects Liability Period (DLP).

11) Inventory of Equipment (if any) - This is a list of installed equipment in


the Project. This shall also include details of the equipment not limited
to the following:

a) Brand
b) Model
c) Location of installation

sequence of operation, modes of Operation, system
interface, control data, start-up, running and shutdown
procedures, interlock between plant items, operating
instruction for standby plant including details of any
automatic control system and precautions necessary to
overcome known hazards.

defect to be look for, special tools, test equipment and
ancillary services, maintenance, inspection, modification
information, lubrication schedule and periodic maintenance
task / troubleshooting.

c) Equipment Date and Parts Identification

c.1) Equipment Data

Includes the type, model and serial number of each


equipment including capacities (e.g. horsepower, speed,
rated current, etc.), utilities (power supply, water supply,
etc.) and options (e.g. Casing IP rating, insulation class,
etc.:) include the name of the manufacturer / suppliers.

c.2) Parts identification and recommended spares

This is the list detailing the replaceable equipment, quantity,


assemblies and components. These include the name of the
manufacturer / suppliers and model numbers.

d) Manufacturer’s Literature / Brochures

This shall consist of the complete set of the Manufacturer’s


Literature, catalogues, brochures, product description and
performance details with each application.

6.11.2 If there are insufficient data / information found, the Project QA/QC Head
shall notify the Project Manager or Project in-charge to conform with the
requirement. The Project Manager or Project in-charge shall coordinate with
the responsible parties to correct or satisfy the identified deficiency.

6.11.3 Once all necessary documents are found to be sufficient, the Project QA/QC
Head shall recommend submission to the Owner or Operator. Records of
acceptance of these relevant documents shall be recorded using the
Inspection Checklist Form and stored in accordance with the Control of
Record Process.

6.12 Design Process Quality Audit

The sequence of activities is summarized in the Flow Chart at Appendix 10. The
following supplementary information shall also apply:

6.12.1 Responsibility - The Corporate QA/QC Head has overall responsibility for the
implementation of an effective Design Process Quality Audit. Planning and
implementation of the audit plan shall be performed under the guidance of
the Corporate QA/QC Head who will have the full support of the SBU QA/QC
Manager.

6.12.2 Audit Team - The SBU QA/QC Manager shall act as the Audit Team Leader
(ATL) to conduct the Design Process Quality Audit. In the absence or

being audited.

6.12.3 Timing, Frequency and Scope - The Design Quality Audit shall be conducted
at the end of the Detailed Engineering Design Phase or Developed Design
Phase whichever is applicable.

The appointed Design Manager shall notify the SBU QA/QC Head at least
seven (7) working days prior to the end of the Detailed Engineering Design
Phase (DED) or Developed Design Phase (DD).

The scope of the Design Process Quality Audit shall cover not limited to the
following process stages:

1) Appointment of Design Manager


2) Appointment of Job Captain
3) Analysis of Bid Invitation and Approval to Proceed
4) Selection of Design Teams
5) Orientation of Design Teams
6) Designing
7) Evaluation of Designs
8) Turnover to Estimating Team – see Bid and Estimate Procedure
9) Deliberation

6.12.4 Audit Planning – The Audit Team headed by Audit Team Leader (ATL) shall
prepare a Design Process Quality Audit Plan and Checklist. The SBU QA/QC
Manager shall review and approve the Design Process Quality Audit Plan and
Checklist prepared by the Audit Team prior to the conduct of the audit.

6.12.5 Audit Schedule and Notification – The Audit Team Leader (ATL) shall notify
in writing the Technical Services Head through the appointed Design
Manager of the audit schedule and agenda, at least three (3) working days
prior to the conduct of the audit. The Audit Team Leader (ATL) shall

6.12.6 Audit Conduct - The audit shall proceed in accordance with the Design
Process Quality Audit Plan. As the need arises, the Audit Team Leader (ATL)
may deviate from the plan when initial findings in the ongoing audit reveals
that there is a need to properly cover some additional audit scope not
included in the original Design Process Quality Audit Plan and Checklist.

6.12.7 Post Audit Meeting – At the end of the audit, the Audit Team Leader (ATL)
shall conduct a Post Audit Meeting with the Technical Service Head and the
appointed Design Manager to discuss the audit findings and observations.
The Audit Team Leader shall issue an Initial Design Process Quality Audit
Report during the Post Audit Meeting.

The Audit Team Leader (ATL) with the approval of the SBU QA/QC Manager
shall issue Observation Report and/or Corrective Action Report (CAR) as
necessary to the appointed Design Manager in accordance with the
Corrective Action Process.

6.12.8 Audit Reports - The Audit Team Leader shall furnish a copy of the Final
Design Quality Audit Report within three (3) working days after the conduct
of the audit to the following:

1) Head of Technical Services


2) Design Manager
3) SBU Head

6.12.9 Corrective Actions – The appointed Design Manager shall propose corrections
or corrective actions to address the nonconformity observed in accordance
with the Corrective Action Process. The appointed Design Manager under the
supervision of the Technical Services Head shall ensure that the required
corrections and/or corrective actions are completed within the specified time
and in accordance with the Corrective Action Process. The disposition of the

closure of nonconformity shall be in accordance with the Corrective Action
Process.

6.12.11 Unscheduled Audit – Unscheduled Special Design Process Quality Audit may
be conducted at the direction of the SBU QA/QC Manager if the following
conditions exist:

1) Indication/s of a major design flaw was observed


2) Re-occurrence of previous or new nonconformity that needs immediate
attention
3) Previous audit of an item indicates the need for follow-up.
4) Period of time established for implementing corrective action has
elapsed.

6.13 Bid Process Quality Audit

The sequence of activities is summarized in the Flow Chart at Appendix 11. The
following supplementary information shall also apply:

6.13.1 Responsibility - The Corporate QA/QC Head has overall responsibility for
the implementation of an effective Bid Process Quality Audit. Planning and
implementation of the audit plan shall be done under the direction of the
Corporate QA/QC Head who will have the full support of the SBU QA/QC
Manager.

6.13.2 Audit Team - The SBU QA/QC Manager shall act as the Audit Team Leader
(ATL) to conduct the Bid Process Quality Audit. In the absence or
unavailability of the SBU QA/QC Manager, the Corporate QA/QC Head shall

of Engineering and the Job Captain to ensure that the audit will not unduly
affect other ongoing bid and estimate activities.

6.13.6 Audit Conduct - The audit shall proceed in accordance with the Bid Process
Quality Audit Plan. As the need arises, the Audit Team Leader (ATL) may
deviate from the plan when initial findings in the ongoing audit reveals that
there is a need to properly cover some additional audit scope not included
in the original Bid Process Quality Audit Plan and Checklist.

6.13.7 Post Audit Meeting – At the end of the audit, the Audit Team Leader (ATL)
shall conduct a Post Audit Meeting with the Head of Engineering and Job
Captain to discuss the audit findings and observations. The Audit Team
Leader shall issue an Initial Design Process Quality Audit Report during the
Post Audit Meeting.

The Audit Team Leader (ATL) with the approval of the SBU QA/QC Manager
shall issue Observation Reports and/or Corrective Action Reports as
necessary to the Job Captain in accordance with the Corrective Action
Process.

6.13.8 Audit Reports - The Audit Team Leader shall furnish a copy of the Final
Design Quality Audit Report within three (3) working days after the conduct
of the audit to the following as necessary:

1) Head of Engineering
2) Job Captain
3) SBU Head

6.13.9 Corrective Actions – The Job Captain shall propose corrections and
corrective actions to address the nonconformity observed in accordance
with the Corrective Action Process. The Job Captain under the supervision
of the Head of Engineering shall ensure that the required corrective actions

closure of nonconformity shall be in accordance with the Corrective Action
Process.

6.13.11 Unscheduled Audit – Unscheduled Special Bid Process Quality Audit may be
conducted at the direction of the SBU QA/QC Manager if the following
conditions exist:

1) Indication/s of an imminent bid disqualification was observed


2) Re-occurrence of previous or new nonconformity that needs immediate
attention
3) Previous audit of an item indicates the need for follow-up
4) Period of time established for implementing corrective action has
elapsed

6.14 Project Quality Audit

6.14.1 Responsibility - The Corporate QA/QC Head has overall responsibility for
the implementation of an effective Project Quality Audit. Planning and
implementation of the audit plan shall be done under the direction of the
Corporate QA/QC Head who will have the full support of the SBU QA/QC
Manager.

6.14.2 Audit Team - The SBU QA/QC Manager shall act as the Audit Team Leader
(ATL) to conduct the Project Quality Audit. In the absence or unavailability
of the SBU QA/QC Manager, the Corporate QA/QC Head shall appoint a
Head Office Quality Assurance (QA) Personnel to act as the ATL. The Audit

6.14.4 Audit Planning – The Audit Team headed by Audit Team Leader (ATL) shall

Manager shall review and approve the Project Quality Audit Plan and
Checklist prepared by the Audit Team prior to the conduct of the audit.

6.14.5 Audit Schedule and Notification – The Audit Team Leader (ATL) shall notify
in writing the Project Manager through the Project QA/QC Head of the audit
schedule and agenda, at least three (3) working days prior to the conduct
of the audit. The Audit Team Leader (ATL) shall coordinate with the Project
Manager and the Project QA/QC Head to ensure that the audit will not
unduly affect other ongoing project activities.

6.14.6 Audit Conduct - The audit shall proceed in accordance with the Project
Quality Audit Plan. As the need arises, the Audit Team Leader (ATL) may
deviate from the plan when initial findings in the ongoing audit reveals that
there is a need to properly cover some additional audit scope not included
in the original Project Quality Audit Plan and Checklist.

6.14.7 Post Audit Meeting – At the end of the audit, the Audit Team Leader (ATL)
shall conduct a Post Audit Meeting with Project Manager and the Project
QA/QC Head to discuss the audit findings and observations. The Audit Team
Leader shall issue an Initial Project Quality Audit Report during the Post
Audit Meeting.

The Audit Team Leader (ATL) with the approval of the SBU QA/QC Manager
shall issue Correction Action Report as necessary to the Responsible Entity
(RE) in accordance with the Corrective Action Process.

6.14.8 Audit Reports - The Audit Team Leader shall furnish a copy of the Final
Project Quality Audit Report within three (3) working days after the conduct
of the audit to the following:

1) Project Manager
2) Project QA/QC Head
3) SBU Head

6.14.9 Corrective Actions – The Responsible Entity (RE) shall propose corrections
and corrective actions to address the nonconformity observed in
accordance with the Corrective Action Process. The Responsible Entity (RE)
under the supervision of the Project Manager shall ensure that the required
corrections and corrective actions are completed within the specified time
and in accordance with the Corrective Action Process.

6.14.10 Effectiveness of Corrective Action/s and Closure of Nonconformity - The


Audit Team shall conduct a review of the effectiveness of the corrective
action/s taken in accordance with the Corrective Action Process. Likewise,
closure of nonconformity shall be in accordance with the Corrective Action
Process.

6.14.11 Unscheduled Audit – Unscheduled Project Quality Audit may be conducted


at the direction of the SBU QA/QC Manager if the following conditions exist:

1) Commencement of major/special scope of work.

6.15 Disputes and Resolution

6.15.1 In the event that there is uncertainty or disputes regarding the application
or interpretation of the quality assurance and quality control process in the
Project Level, these shall in the first instance be submitted by the Project
QA/QC Head to the SBU QA/QC Manager for resolution.

6.15.2 In the event that there is uncertainty or disputes regarding the application
or interpretation of the quality assurance and quality control process in the
SBU Level, these shall in the first instance be submitted by the SBU QA/QC
Manager to the Corporate QA/QC Head for resolution.

6.15.3 In the event that there is uncertainty or disputes regarding the application
or interpretation of the quality assurance and quality control process in the
Corporate Level, these shall in the first instance be submitted by the
Corporate QA/QC Head to the TQM Manager for resolution.

6.15.4 In the event there is no resolution, the issue will be escalated to the
President for a final decision.

6.16 Amendments, Improvements and Special Conditions

6.16.1 The Corporate QA/QC Head shall conduct an annual review of the QA/QC
Process with the members of the QA/QC Section. Improvements and
amendments shall be implemented and recorded accordingly to ensure the
effective implementation of the QA/QC Process.

6.16.2 The Project QA/QC Head, SBU QA/QC Manager or the Corporate QA/QC
Head may deviate from the normal implementation of the standard
procedure described in this process where immediate solution is vital to the
resolution of a problem during the following conditions:

1) Emergency
2) Unsafe Working Conditions
3) Significant Loss (Financial, Opportunities)
4) Environmental Catastrophe
5) Noncompliance to Statutory, Regulatory and/or Legal Requirememts

6.16.3 The Project QA/QC Head, SBU QA/QC Manager and Corporate QA/QC Head
shall record in writing the immediate actions taken and their results. This
shall be discussed and reviewed with relevant authorities when the above
conditions and their risks are already fundamentally controlled or
significantly addressed with.

7.0 RISK MANAGEMENT

7.1 The primary purpose of the Quality Assurance / Quality Control activities is to
ensure that works and processes are being implemented according to the set
standards. There are two (2) key aspects related to the risk management process:

7.1.1 Risk that the Quality Assurance / Quality Control process is flawed or not
applied consistently and correctly.

7.1.2 Risk that the processes do not achieve their objectives, which is to ensure
our products and services are in accordance with our expectations.
