Colostomy Care

Steps 4 3 2 1 Remarks

1. Assess the ostomy site and the fullness of the pouch.


Determine the need for an appliance change, type, and size of
appliance. Select an appropriate time to change the appliance.

2. Assemble the supplies and equipment needed.

3. Introduce yourself and verify the client’s identity

4. Perform hand hygiene.

5. Don clean gloves.

6. Provide client privacy & assist the client to a comfortable


position.
7. Empty and remove the ostomy skin barrier.

8. Clean and dry the peristomal skin and stoma.

9. Assess the stoma and peristomal skin.

10. Place a piece of tissue or gauze over the stoma.

11. Prepare and apply the skin barrier.

12. Dispose the used supplies.

13. Remove gloves and perform hand hygiene.

14. Document the amount, consistency, and color of the stool.

Scoring: 4 - Excellent, 3 - Good, 2 - Fair, 1 - Poor, 0 - Not Done
