SUBJECT: NEW APPOINTMENT OF MEMBERS OF THE DISCIPLINE BOARD

Madam/Sir:

I would like to duly appoint you as the members of the Discipline Board of this
institution as the requirement of CHED’s Memorandum Order No.09 series of 2013 in
section 22.2 states that “A discipline committee shall be established in all HEI’s to
ensure due process in dealing with student conduct”.

The Discipline Board consists atleast of the following:

a. School Administrator – Chairperson

b. Prefect Of Discipline – Assistant Chairperson
c. The Academic Dean of the College under which subject student belongs
d. Student Affairs Officer
e. Guidance Counsellor/Personnel
f. Faculty representative
g. A representative from Student Council
h. Parent/s
i. Any other designated school official as may be determined or appointed by
the School Administrator.

The Disciplinary procedure:

1. The Head of Student Affairs, upon receipt of the written (formal) complaint or
report on a major and grave offense, shall determine whether such report or
complaint warrants formal investigation by the Discipline Board or not.

2. If formal investigation is deemed necessary, the Head of Student Affairs through

the Prefect of Discipline, recommends to the School’s Administrator the
Assembly of the Discipline Board, the members which are duly appointed by the
School’s Administrator.

Through creating this, we can create timely mechanisms to address student grievances .
