ASSESMENT SKILL SCORE REMARKS


1. Explains the procedure to the client.
2. Instructs the client to empty her bladder.
3. Positions the client supine, knees slightly flexed with
small pillow or rolled towel under one side.
4. Washes hands using warm water.
5. Observes the woman's abdomen for longest diameter
and where fetal movement is apparent.
6. Places both hands on her abdomen.
7. Palpates the superior surface of the fundus.
8. Determines the mobility, shape and consistency.
SECOND MANEUVER
9. Faces the client.
10. Places the palms of each hand on either of the
abdomen.
11. Palpates the sides of the uterus.
12. Holds the left hand stationary on the left side of the
uterus while the right hand palpates the opposite side of
the uterus from top to bottom.
13. Holds the right hand steady.
14. Repeats palpation using the left hand on the left side.
THIRD MANEUVER
15. Facing the client, gently grasps the lower portion of the
abdomen just above the symphysis pubis between the
thumb and index finger and tries to press the thumb and
finger together.
16. Determines any movement and whether the part is firm
or soft.
FOURTH MANEUVER
17. Facing the foot part of the client, places fingers on both
sides of the uterus approximately 2 inches above the
inguinal ligaments, pressing downward and inward in
the direction of the birth canal

18. Allows fingers to be carried downward.
