MICHAEL JAYSON H. SY, Filipino, of legal age, with residence and postal
address at Brgy. San Roque, Marikina City, herein referred to as the
“ASSIGNEE”.
-WITNESSETH that-

WHEREAS, the ASSIGNOR is the registered owner of a motor vehicle,

particularly described as follows:

WHEREAS, the ASSIGNOR has offered to assign all his rights, title and
interest over the above motor vehicle and the ASSIGNEE hereby accepts the
assignment in accordance with the terms herein set forth;
NOW, THEREFORE, for and in consideration of the foregoing premises,
which the ASSIGNOR hereby acknowledged, the ASSIGNOR hereby assigns,
transfers and conveys unto the ASSIGNEE, all his rights, title and interest to
the aforementioned motor vehicle and the ASSIGNEE by these presents
hereby accepts the assignment.
IN WITNESS WHEREOF, the parties have hereunto set their hands on the

Rights of a Motor Vehicle and known to me to be the same persons who
executed the foregoing and exhibited to me their Government issued IDs
above-mentioned and competent evidence of identities and acknowledge to
that the same are their own free and voluntary act and Deed in accordance
with the 2004 Rules on Notarial Practice.

This instrument consisting of three (2) pages including this page upon
acknowledgement is written and had been signed by the parties and their
instrumental witnesses on each and every page thereof and refers to a Deed
of Assignment and Transfer of Rights of a Motor Vehicle.
