1 . ln compliance to the mandatory filing/submission of Certificate of Compensation

paymenUTax Withheld (BlR Form No. 2316), this Office hereby informs all Public
School District Supervisors and Elementary/Secondary School Heads to facilitate
the distribution and submission of hard and soft copies of the signed BIR Form
2316.

2. Said forms must be properly filled-up and signed by employees and retum the two
(2) copies to the SDO Accounting section for signature. District shall retrieve the
signed forms and distribute one (1 ) copy to the employees and one (1) copy must
be scanned and submitted to SDO.

3. Upon submission of softcopies of the BIR Form No.2316, thefollowing procedures

shall be strictly observed:

a. Scan the signed BIR Form No. 2316 through scanning machine or device.
b. Store the sofr copies of BIR Form 2316 using the 'PDF" fi16 format with the
filenames alphabetically arranged in a Universal Storage Bus (USB) memory
stick. The filename shall contain the following information:

. Surname of the employee

o Taxpayer ldentification Number (TlN) of the employee; and
r Taxable period

Example: Dela Cruz_4607 1 3/,2G000_12312021

4. lt is hereby advised to observed process for submission of the signed and scanned
forms:

a. Elementary Schools - to be submitted by distrid

b. Secondary Schools - to be submitted by School
c. Division Office - to be submitted at Accounting Sec'tion

5. The BIR Form No. 2316 together with soft copies shall be submitted to the SDO
on or before February 1E,2022.

6. For immediate dissemination and compliance.
