    Dear Ma’am,

    On the day of the exam, I encountered an unforeseen and unavoidable situation that
    prevented me from attending. May 19-22, 2023 I was admitted to Coop Hospital and
    received medical care for Hypokalemia, Insomnia secondary to Anxiety Reaction. The
    doctors determined that my condition required immediate attention and monitoring,
    necessitating an extended hospital stay. I can provide you with the relevant medical
    documentation/certificate if required.

    I kindly request your assistance in arranging a makeup exam so that I may


    demonstrate my knowledge and competency in the course. I am willing to provide any
    necessary documentation or evidence to support my request. Thank you for your
    understanding and consideration of my circumstances. I highly value my education and the
    opportunity to demonstrate my knowledge and skills.
