MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This Memorandum of Agreement is entered into and executed by and among:

Holy Trinity University an educational institution located at Quezon Street, Brgy.


Maligaya, Puerto Princesa City represented herein by SR. ELENITA B. OCAMPO, O.P. in her
capacity as the president of the Holy Trinity University, who is duly authorized to represent it in
this transaction, herein referred to as the UNIVERSITY;

The PROVINCIAL ENVIRONMENT AND NATURAL RESOURCES OFFICE (PENRO), a


government agency with office address at Sta. Monica, Puerto Princesa City represented herein by
FELIZARDO B. CAYATOC in his capacity as the Head Officer, herein referred to as the AGENCY

-and-

MARIA ELLA BASBAS and BENEDICT D, ORLIDO JR, 3 RD YEAR BS GEODETIC


ENGINEERING, bonafide Filipinos and with residential address at Puerto Princesa City, herein after
referred to as the STUDENT-TRAINEES.

PURPOSE

The purpose of this Memorandum of Agreement is to establish an agreement among the


HOLY TRINITY UNIVERSITY, the PROVINCIAL ENVIRONMENT AND NATURAL RESOURCES
OFFICE (PENRO), and MARIA ELLA BASBAS and BENEDICT D, ORLIDO JR concerning the design,
implementation, and monitoring of the On-the-Job Training Course of the HOLY TRINITY
UNIVERSITY.

This MOA seeks to coordinate, facilitate and expedite the implementation of the said
program for the exclusive benefit of the above-mentioned parties.

PARTIES
-WITNESSETH-

WHEREAS, the HOLY TRINITY UNIVERSITY, is requiring the On-the-Job Training to


qualified students who will undergo industry exposure prior to graduation in order to become
adequately familiar to the actual industrial operations and management, thereby augmenting their
formal training;

WHEREAS, the PROVINCIAL ENVIRONMENT AND NATURAL RESOURCES OFFICE


(PENRO), agreed to accept the students of the HOLY TRINITY UNIVERSITY as trainees in support
of its On-the-Job Training Program which aims to strengthen the academe-industry linkage and
complement existing course curricula to match industry demand;

WHEREAS, MARIA ELLA BASBAS and BENEDICT D, ORLIDO JR are willing and able to
undergo training at the PROVINCIAL ENVIRONMENT AND NATURAL RESOURCES OFFICE (PENRO)
to further develop his/her skills and competency.

NOW, THEREFORE, in consideration of the above cited premises and their mutual
covenants hereinafter set forth, the parties do hereby agree on the following TERMS AND
CONDITIONS:

1. Shall recommend qualified students who will undergo the training program taking into
consideration the requirements of the Provincial Environment and Natural Resources Office
(PENRO) in terms of qualifications and number of students;
2. Shall orient the Student-Trainees about the rules, proper attitude and behavior inside the
Agency;
3. Shall coordinate training program with the the Provincial Environment and Natural
Resources Office (PENRO);
4. Shall submit to the Provincial Environment and Natural Resources Office (PENRO)the
documents required for this program;
5. Shall request the parent/s or the guardian of the student-trainees to sign a waiver
renouncing any claim against the Provincial Environment and Natural Resources Office
(PENRO) for any injury that the student-trainees may sustain or loss that they suffer,
personal or pecuniary, arising from negligence in the performance of their duties and
functions while under training;
6. Shall assist the Agency in monitoring the active participation and attendance of the student-
trainees; and
7. Shall provide the student-trainees and the Agency with the guidelines for evaluating the
student-trainee’s performance in the internship program.

Provincial Environment and Natural Resources Office (PENRO)

1. Shall deploy the student-trainees to the sections of the Agency related to his/her field from
April 25 to June 3, 2022 with a minimum of 240 hours unless extended upon agreement by
the school and the Agency;
2. Shall provide experience to the student-trainees that will enhance his/her ability to work as
a professional of its field in the future, and expose the students to how operations are done
in the workplace;
3. Shall assign a supervisor who will guide and instruct the trainees on what tasks to do;
4. Shall provide the trainees with an identification card and a time-card for recording
attendance;
5. Shall provide orientation about the Agency and office policies;
6. Shall not be liable for any accident or body injuries occurring to the student/trainee and for
any loss of student-trainee’s personal property while in the premises of the Agency for the
duration of the training;
7. Shall inform the University if there is a concern regarding the student-trainee; and
8. Shall issue Certificate of Completion to the student-trainee upon fulfilment of all training
requirements.

STUDENT-TRAINEE

1. Shall submit to the Agency the following requirements before the start of the training:
a. Letter of Recommendation from the School
b. Personal Data Sheet (2 copies)
c. Parent/ Guardian’s Consent/ Waiver Form
d. Vaccination ID/Card
2. Shall observe the existing office decorum for the duration of the training/program;
3. Shall shoulder all expenses, including travel and transportation expenses for the duration of
the training program;
4. Shall use the Daily Time Record (DTR) provided by the Human Resource Management
Section of the Agency in logging in and out of the office;
5. Shall inform the designated supervisor in advance if he/she is not reporting to the office,
and shall make up for the days absent to complete the required number of hours of training;

6. Shall be liable for any breakage and/or destruction of any equipment or laboratory facilities
resulting from his/her own doing during the duration of training; and
7. Shall submit an accomplishment report to the Human Resource Management Section of the
Agency on a monthly basis signed by the designated supervisor.

This contract shall not create an employer-employee relationship and the student-trainee will not
be entitled to benefits enjoyed by the regular PENRO personnel. Furthermore, the services
rendered hereunder are not considered as government service.

DURATION

This Agreement shall take effect upon signing hereof by the parties. Any parties may withdraw
from this Agreement provided there is a due and proper notice. The entire Agreement may be
terminated any time upon mutual agreement by the parties.

ACKNOWLEDGMENT

IN WITNESS WHEREOF, the parties hereto have signed this presence on this _____ day of
