JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

Valenzuela City, after having been sworn in accordance with the law hereby depose and state:

1. That we personally know PAOLO RAY C. ANTONIO and JENNIE ROSE C.

DANTE, both of legal age, Filipino, single (living together) who have been our long-time
neighbors and acquaintances residing at 30 F. Lucero St., Viente Reales, Valenzuela City,
Philippines;

2. That they have been in a common law relationship since_________ and up to present;

3. That being in a common law relationship for more than ______(_____) years now, they
have lived together under one roof as we call it their home for right from the start of their
relationship which usually refer to as live-in partnership;

4. That we are not related to the aforementioned person within the fourth civil degree, either
by consanguinity or affinity, and we do not have any pecuniary interest in this matter;

5. That having no any legal impediment to marry each other, it is now their desire to
legalize their relationship and enter into a marriage as they are both ready to undertake
their obligations and share the blessings of a married life as a couple;

6. We are executing this affidavit to attest to the truthfulness of the foregoing statements and
for all legal intents and purposes it may serve.
