AFFIDAVIT OF DISCREPANCY

I, CRISTHEL E. ESCUADRO, Filipino, of legal age and presently residing at Jalandoni, Brgy.
Oringao, Kabankalan City, Negros Occidental, after having been duly sworn in accordance with law
hereby depose and states:

1. I am the Legitimate daughter of Spouses Dante E. Escuadro and Lilibeth Elentorio,

having been born on 20 October 2000 in Bayana, Bacoor, Cavite. Xerox copy of my
Certificate of Live Birth is hereto attached as Annex “A” and Certificate of Marriage of
my parents attached as Annex “B’’.
2. In my Certificate of Live Birth in Entry No. 1, my full name was erroneously entered as
“CRISTHEL ELENTURIO” instead of true/correct spelling “CRISTHEL ELENTORIO
ESCUADRO”.
3. Likewise, in my Certificate of Live Birth in Entry No. 6, the name of my Mother was
erroneously entered as “LILIBETH BAJOS ELENTURIO” instead of the true/correct
spelling “LILIBETH BAJOS ELENTORIO”. Xerox copy of my Mother Certificate of
Live Birth is hereto attached as Annex “C.”
4. Furthermore, in Entry No. 13 my father’s name was erroneously entered as “DANTE
COBOL ESQUADRO” instead of true/correct spelling “DANTE COBOL
ESCUADRO”. Xerox copy of my Father Certificate of Live Birth is hereto attached as
Annex “D”.
5. The name of my father “DANTE COBOL ESCUADRO” was entered in my Certificate
of Live Birth with his signature in Entry No. 20, however this not appeared in my
registered name.
6. Ever since my Elementary/Junior High/ Senior High/College Education and/or other
documents I have been using and/or have used the name “CRISTHEL ELENTORIO
ESCUADRO”. My diplomas are here to attached as Annex “E”, “F”, “G”, and “H”.
7. The name “CRISTHEL ELENTURIO” and “CRISTHEL ELENTORIO
ESCUADRO” belongs to one and the same person. Moreover, the names “LILIBETH
BAJOS ELENTORIO” and “LILIBETH BAJOS ELENTURIO” and, also “DANTE
COBOL ESCUADRO” and “DANTE COBOL ESQUADRO” belong to one and the
same person.

I am executing this Affidavit of Discrepancy to attest under oath the truth of the
foregoing and for all legal purposes and intents it may be worth.
