This is to certify that MARILYN AQUINO ZACARIAS born on January 27, 1996 in
Montalban, Rizal and JOHN ZEL LAGUIDAO BELLEN born on March 10, 1975 in San
Carlos, Pangasinan and have been living as spouses in good faith, taking on all of the task and
responsibilities that follow with being in the said relationship cohabiting in the same household
at 198 Sitio Libis, Barangay Rosario, Rodriguez Rizal since year 2014. The latter is currently
imprisoned at Marikina City Jail.

This certification is being issued upon the request of the above-mentioned person as part
of the requirement before visitation with the common-law-husband.
