Dear Stephen Smith

Your recent blood test has shown that you are at risk of developing type 2
diabetes. I recommend that you attend the course below and have referred
you. The course aim is to reduce your risk of getting type 2 diabetes.

The NHS Diabetes Prevention Programme (NDPP) service is free to all

those at risk of Type 2 diabetes registered with a local GP

Participants under the NDPP programme will be offered the opportunity to

access Information and support to make lifestyle changes to prevent
progression to type 2 diabetes have full access to all our digital tools
(desktop and App tools) including handbooks in 16 different languages

The programme includes;

 Tailored Type 2 diabetes curricula

 One to One face to face support
 Access to over a 1000 recipes
 Culturally Tailored remote group sessions
 Local Community groups dedicated to Type 2 diabetes
 Meal builders
 Access to tools to improve mental wellbeing via Headspace

You will also need an annual blood test checking for check your sugar level
(called Hba1c) this checks your blood sugar level over the last 3 month. If
your Hba1c goes above 48 then you will be deemed to have diabetes. The
Surgery will contact you to arrange this test once a year.
