PATIENT 1

COULD YOU EXPLAIN WHICH FACTORS CHANGED THE INITIALLY SUSPECTED CONDITION TO THE FINAL
DIAGNOSIS OF ILD

1 First of all, the patient is a smoker.


2 We can view that this is one of the risk factors in developing IPF.
3 Moreso, the patient presented comorbidities.
4 These includes sleep apnea and pulmonary embolism.
5 So we can view that also these comorbidities affected the patients overall health condition.
6 Secondly, his occupational exposure risk also causes such things.
7 We suspect that the patient has been longly exposed to substances from his workplace.
8 This, also changed our perspective that his condition is not just any other common respiratory disease.
9 At the time of diagnosis, the patient is already presenting serious symptoms.
10 Such as coughing up blood, pain the the chest and becoming restless.
11 You see, from the initial consultation, the patient's symptoms becomes uncontrollable.
12 His lung function is deteriorating.
13 So by that time, we had to take a step on our previous treatment.
14 Since his breathing is becoming worst, we have to switch to a much serious treatment.

HOW WOULD YOU RATE THE OVERALL JOURNEY FOR THIS PATIENT BETWEEN DISEASE ONSET AN
DIAGNOSIS IN TERM OF BEING EFFICIENT, QUICK AND DEFINITIVE
1 To start with, I can say it was such a difficult journey for him.
2 While he presented comobidities, he also developed such serious symptoms.
3 By that time, we already decided to run series of tests.
4 These tests confirmed that he developed IPF.
5 I can remeber him being so depressed when we told him about the result.
6 He was also anxious and can't focus.
7 I think it affected not only his physical but also his mental and emotional health.
8 Controlling the symptoms was also challenging since it's serious.
9 However, with all the efforts we are doing, I can say he is likely recovering from it.
10 I can say that the process is slow, but at least, he has chances of recovering.
11 So overall, there's a major hurdle.
12 Nonetheless, he is improving.

HOW WOULD YOU RATE THE OVERALL TREATMENT FOR THIS PATIENT BETWEEN DIAGNOSIS AND
CURRENT TREATMENT?
1 Thorough assessment was done since he first consulted me.
2 Series of tests was done.
3 We started from October 2018 and he is still currently under treatment.
4 I often see him between 1-2months.
5 So from that 2 years since the onset of disease, I can say that there's a major improvement.
6 All these started on treating the risk factors.
7 We adviced the patient to cease smoking.
8 He says it is hard since it became part of his living.
9 However, gradually, he is practicing to quit smoking.
10 As of the moment, to my knowledge, he already put an end to it.
11 Aside from treating the risk factors, we also have clinical treatment.
12 We prescribed certain medicines to control such serious symptoms.
13 By the time he is still smoking, the prescriptions are not yet to be effective.
14 However, when he gradually decreases his smoking habit, medicines starting to take effects.
15 Up until today, we are still providing him the support he needed.

16 Not only physically, but mentally and emotionally.


17 So I can say that we are doing a positive outcome on his health.
18 These are all because of the the cooperation of each other to the regime.
