SUBJECT : Spot Report re Vehicular Incident

DATE : August 20, 2023

1. Spot report on reckless imprudence resulting to physical injuries and damage of property.
That on or about 10:30 o’clock in the afternoon of Augustl 20, 2023 along the National Highway
located at El Centro, Brgy. Poblacion,Iligan Cityl. Initialinvestigation was conducted by the
elements of this station revealed that on above-mentioned time, place and date, while Honda
Click 125cc black and white combinationbearing plate no. 85124 registered under the name of
Louie Tebalan, driven by one Melchor Tayone y Christopher, 23 years old, married, a habal-
habal driver and a residence of Brgy. Del Carmen, Zone 1-B, Iligan Cityl holder of Non-
professionl driver’s license no. GO3-11-002351 expires on 2023-11-07, with back rider Rexter
Minoc, of Brgy. Digkilaan, Hinaplanon, Iligan City was running heading towards Brgy. Poblacion
this municipality accidentally bumped to the left side mirror of Toyota Hilux 4WD, color white
bearing plate no. KCU-2623 registered under the name of Danjie Milburn, Isabel village, Pala-o,
Iligan City, driven by James y Bobs , 26 years old, married, Delivery truck driver and a
residenceof Suarez, Zone Saturn, Iligan City not carried driver license, traversing in opposite
direction (from Brgy. Tominobo headed to Brgy. Poblacion this municipality). The driver and the
back rider sustained injuries at different parts of body and were rushed to a hospital in Pala-o
City Hospital for medical treatment while the driver of delivery truck and said motorcycle were
brought to this office for documentation and, for proper disposition. The driver and the back rider
of said motorcycle were not wearing crash helmet. Any development, progress report will follow.
