I hereby apply as a vendor for the 2023 Cush Ministries Benefit Concert and Vendor Fair.
Operations will be Saturday March 25th, 2023 from 4:00 PM to 8:00 PM. This will be a one day
event and set up will open at 2:00. $25 booth fee for non-food vendors is due immediately to
reserve your space, check made payable to Cush Ministries International or via PayPal at
www.PayPal.me/cushministries. Food vendor rate is based on sales. Please reach out for more

RULES OF THE MARKET

1. If food is to be sold, business licenses and food handlers permits must be provided.
2. All vendors are responsible for liability insurance and compliance with any and all
City/County/State business requirements
3. Vendors are responsible for leaving a clean area at closing
4. All vendors must exhibit professional behavior at all times
5. All booths must fit in a 10’x10’ space; vendors are responsible for providing their own
canopy (if desired) and tables for set up.
