Rescue and Evacuation Plan

Work Location: Tacora Resources Thickener Tank

Task: Thickener Tank Overhaul/ Rebuild

Hazard Potential: Injured or severely ill employee, fire, evacuation, power


outage, fall from scaffold, injuries causing mobility issues.

Control Methods:

• In-depth review of risk assessment for all employees and sub-contractors.


• Site specific orientation.
• Tacora take 5 Program strictly enforced and monitored.
• Strong focus on housekeeping throughout project.
• Ensure compliance with Tacora Safety Standards
• All employees will carry a flashlight/headlamps

Rescue Plan:

• The first person on the scene should:


· Stay calm
· Perform Scene survey
· Commence emergency first aid or CPR as required.
· Activate ERT according to the following protocol (if needed).

• In the event of an emergency; the first person on the scene will call for help
by one of the following methods:

· A cellular phone may be used in areas of sufficient signal strength


dial 911 for Fire/Emergency Services

The individual contacting Fire/Emergency Services shall identify his/herself state the
nature of the emergency, giving the location for the response team to respond. The
_____________________ is the location to direct the emergency response team.
Do not terminate contact with ERT until they tell you to do so.

• Supervisor will appoint an employee to meet and direct ERT to the scene of
the emergency. The employee will be located at ________________, ERT
will assess the situation and take command/supervise emergency
operations.

Employee Fall from Scaffold:

• Contact 911, remain calm, answer all questions asked by 911 dispatcher,
do not hang up phone until told to do so by 911 dispatcher. Provide name,
location, number of those injured and mechanism of injury if known.
• Contact Tacora Security at 987-9625, let them know 911 has been
dispatched and will be arriving to the gate
• Secure the area immediately around the fallen person to prevent
secondary injury to passersby.
• Keep curious onlookers away. They only add additional distress to the
responsive casualty.
• Gather and safeguard any material/tools/personal effects that may have
fallen to the floor below the suspended person.
• Tend to the fallen employee, perform scene survey
• Do not move the individual as they may be suffering from a head, neck or
spinal injury or major trauma injuries (internal bleeding)
• Ensure that the employee is only moved if there is a life threatening injury
(ex. Unable to breathe in position found).

Report of a fire in the thickener tank:

If you are the first person at the scene of a fire:


• Stay calm

• Check for danger and conduct scene survey

• Assess incident

• If it is a small fire – extinguish if possible.


Note: Beware of fumes and/or electrical cables

• If you cannot extinguish the fire, make sure the area is clear of personnel,
then retreat from the scene;

• Evacuate to the muster point: Marshall Lunchrooms

• Call for help: PHONE 911 for Fire/Emergency Services


• Ensure a person is available to direct the Emergency Response Team to the
scene of the fire;
• Assist the Emergency Response Team as directed.

Evacuation of thickener tank
• If it becomes necessary to evacuate the plant, all occupants will be notified

• Ensure your work area is secure from possible hazards during your absence if
safe to do so:
o Shut-off welder’s torch at bottles;
o If using lifting device, ensure weight is lowered to a secure surface,
• Close all doors
• Proceed to the muster point. If you notice an employee or visitor who is not
aware of the evacuation, please inform them of the proper steps to follow.

• Remain at the muster point and ensure persons responsible for roll call
identify you.

• Acknowledge presence when your name is called and if your name has been
missed inform the person doing roll call that you are there.

• Do not re-enter the plant until the “ALL CLEAR” has been given by an
authorized personnel.

Employee Medical Emergency:

• The first person on the scene should:


· Stay calm
· Perform Scene survey
· Commence emergency first aid or CPR as required.
· Activate ERT according to the following protocol (if needed).
• Contact 911, remain calm, answer all questions asked by 911 dispatcher,
do not hang up phone until told to do so by 911 dispatcher. Provide name,
location, number of those injured and mechanism of injury if known.
• Contact Tacora Security at 987-9625, let them know 911 has been
dispatched and will be arriving to the gate
• AED (Automatic External Defibrillator) to be located inside Marshall
Lunchroom
• MIL Supervision to assign employee to go to the admin building entrance
to wait for emergency services to arrive and direct them to the emergency
scene.

Removal of Injured Employee from Thickener Tank:


• The first person on the scene should:
· Stay calm
· Perform Scene survey

· Commence emergency first aid or CPR as required.
· Activate ERT according to the following protocol (if needed).
• Contact 911, remain calm, answer all questions asked by 911 dispatcher,
do not hang up phone until told to do so by 911 dispatcher. Provide name,
location, number of those injured and mechanism of injury if known.
• Contact Tacora Security at 987-9625, let them know 911 has been
dispatched and will be arriving to the gate
• MIL to ensure that engineered man basket is available on site and ready
for transport via crane.
• Persons suspected of head, neck or spinal injuries will be immobilized but
not moved via man basket from thickener tank until assessed by an
emergency medical responder.
• If employee is not suffering from a major trauma injury or potential life-
threatening injury but have mobility issues, they will be removed via
engineered man basket.
• MIL to establish pad area around crane to safely land man basket so
injured person can be tended too by EMS and transported to the Labrador
Health Centre for further examination
