    Dear Officer,

    May I request that our Meralco meter be upgraded to carry load per attached Declaration of Load. Our
    house was maintained by our caretaker, Mr. Rodel Lingon for more than 10 years now and used only
    basic appliances. However, my child who has a family of his own will relocate in Cavite and will use said
    house, thus , will be needing various appliances for their comfort.

    Meralco meter is under my late father’s name, MELQUIADES A. GABRIEL , with CAN 1641995247.

    Attached are photocopies of my late father’s death certificate , my birth certificate , my ID, and an
    authority given to Ms. Julieta Lingon, to facilitate the filing and follow up of this request.

    Hoping for your prompt action on this request.
