to the Door steps of every citizen

• Ayushman Bhava- comprises of

– Ayushman Apke Dwar 3.0
• Starts from 17th September

• Creation and Distribution of Ayushman Cards (AB-PMJAY and ABHA Cards)- via “ Ayushman App “

– Ayushman Mela
• Starts from 17th September

• Planning and Organizing Weekly CHC and AB-HWC Mela – through “ HWC Portal “

– Ayushman Sabha
• On 2nd October 2023

• Gram or Urban ward level Sabha led by the VHSNC /Ward Committee / MAS – through “ HWC Portal “

• For distribution of Ayushman cards to all remaining eligible beneficiaries under PM-JAY Scheme,
with saturation mode to ensure no eligible beneficiary is left
• Card Creation using Self-registration using Ayushman App. https://play.google.com/store/apps/details?id=com.beneficiaryapp

• ASHA’s, CHO’s, PHCO’s and HIO’s with the Supervision of PHC Medical Officer Need to
Make sure that All the assigned Population is covered with ABHA card Creation

• Distribution of the Created ABHA cards are Dispensed to the beneficiary through
Panchayat Raj Dept/ULB (Bulk Printing and laminating)

• Planning Details to be Update Prior Mela in https://ab-hwc.nhp.gov.in/home/login

• Week 1 (1st to 8th)

• Non Communicable Diseases and Patient Safety Day

• Week 2 (9th to 15th)

• TB, Leprosy and other Communicable Disease

• Week 3 (16th to 23rd)

• Maternal & Child Health, Nutrition and Patient Safety

• Week 4 (24th to 30th)

• State / District Specific Health Care (Disease wise Endemic Districts )
