On June 10, 2023, during surveillance at UG HZEM- GCP-1 aquifer water well, URPIU found that
contractor L&T has installed 8 inch RTR pipe as per drawing number 8-RW-3103-FE0W03 Rev.00C on
compacted sand bedding layer at ground level without trenching which is a violation of SAES-S-070
Para 9.5 which states that “The depth of the trench shall be dug deep enough to provide a minimum
ground cover above the top of the pipe, as required in Table 3 of this standard, and a ground bed of
at least 150 mm in depth below the bottom of the pipe.” Contractor is advised through SAPMT to
provide the root cause analyses, corrective and preventative action.

On June 11,2023, During site surveillance and RTR pipe jointing inspection at GCP-1 for 8 inch and 6
inch RTR line number 8" RW-3103-1FE0W03 Aquifer water well area. URPIU found that contractor
Larsen Toubro has completed 70 joints and RTR pipeline is exposed to direct sunlight for more than
one month and has not backfilled which is a violation of the contactor approved special process FPI
KSA-MS-UG para 6.8 which states that "pipes joined in the trenches should not be left for long
periods without backfilling as some joints may rupture due to daily expansion and contraction of the
pipes due to ambient movement the joints may be left exposed for an easy visual inspection during
the field hydrotest. These joints must be backfilled immediately after the test to avoid damages."
Contractor is advised to provide the root cause analysis, corrective action and preventive action to
avoid such scenarios again.

Hydrocarbon. During focus assessment FA-14-L&T for storage preservation of RTR Material URPIU
Noticed the Contractor has failed to comply Storage and Preservation as per company Standards i.e
SAES-L-350-, 01-SAMSS-34 and manufacturer standard FPI-KSA-FSV-MS-H-S. The contractor advised
through SAMPT to submit a root cause analysis, corrective and preventive action. Kindly refer FA-14-
L&T for brief description.
