I, OKE EMIKO WILLIAM (M) Christian, citizen of the Federal Republic of

Nigeria, residing at Along Jeddo Road Okpe, in Okpe Local Government Area of
Delta State, do hereby make oath and states as follows:

1. That I am the deponent herein.

2. That I am a customer to Opay Bank Limited with Account Number
9075917287.
3. That My phone got lost with the said Account Number Sometimes
June, 2022.
4. That all effort made to retrieve the said Account Number Proved
abortive hence this affidavit.
5. That I want my New Number 9159246487 be replace with my old
account Number.
6. That my new Account Number is 9159246487.
7. That my funds should be transferred from 9075917287 to
9159246487.
8. That this affidavit becomes necessary for the information of the
Management of Opay Bank, the general public and to whomsoever it
may concern to please to note.

And that I swear to this affidavit in good faith, believing the same to be true
and correct, in accordance with the Delta State laws of Nigeria 2006.
