REVOCATION OF SPECIAL POWER OF ATTORNEY

KNOW ALL MEN BY THESE PRESENTS:

I, ____________________________________, of legal age, Filipino, and residents of

________________________________, after having been sworn to in accordance with law, do hereby
depose and say.

1. That on __________________, I executed a document entitled “Appointment as

Administrator with Special Power of Attorney”, appointing ________________________, of
legal age and a resident of __________________________________, to act as Administrator
over the parcels of land owned by my deceased brother, _________________________, and
giving him the authority to oversee, manage and act as caretaker of the properties of my
brother, with authority to harvest the coconuts and other agricultural produce found thereon;

2. That due to certain acts of said _______________________ in relation to his administration

and management over the subject properties, which resulted to loss of trust and confidence
on my part on him, I have decided to revoke the authority I had given him as stated in the
above-mentioned document;

3. In view of foregoing, I, hereby REVOKE, ANNUL, and make VOID the “APPOINTMENT AS
ADMINISTRATOR WITH SPECIAL POWER OF ATTORNEY”, dated _________________, and all
