That I SHAIRA MIÑAS HERNADEZ, Filipino citizen/s of legal age, married, with
residence and postal address at Sincalan St. Sumulong Extension, Brgy. San Roque,
Antipolo City do hereby name, constitute, and appoint and by these presents do
hereby named, constituted and appointed ARSELETTE SADO CABAUATAN likewise
of legal age, married and with postal address at 421 Cristobal St. Sampaloc, Manila to be
my own and lawful Attorney-in-Fact, for me and in my name, place and stead, to do and
perform any and all of the foregoing acts and things, to wit:

1. To apply and fill out form(s) required to change the marital status of my
Professional Regulation Commission ID from Single to Married;

2. To change my registered name from SHAIRMA Q. MIÑAS to SHAIRA MIÑAS

HERNADEZ;

3. To submit, sign documents in order to carry out the purpose above-mentioned;

4. To pay necessary and incidental expenses in connection with the said documents;

HEREBY GRANTING unto my said Attorney-in-Fact full power and authority to do and
perform any and every act necessary to render effective the power indicated above, as
though I myself, have so performed it, and HEREBY APPROVING ALL that she may do by
virtue hereof with full right of substitution of her person and revocation of this instrument.
