Observation Prior to Interaction: On May 2, 2023, at 10:00 AM— 4 PM in the afternoon. The interview took place at the San
Jose Home Care. The hall was in an open area, surrounded by plants and had good ventilation. It was a bright and beautiful day. The
environment is clean and well kept, one of the reasons for this was it was spacious. The atmosphere was serene and silent. The
patient was well groomed in a green shirt. The patient's posture is in an upright position. The patient is described as an adult male.
He is well-maintained and well groomed, with both fingernails and toenails cut and no unpleasant odor. He is tall, with short hair,
dark brown eyes, facial hair, a fair complexion, and asymmetrical features. He also has tattoos on both of his arms. He is well-
mannered and makes terrific eye contact. The patient has a normal affect. The patient was eager to respond to the questions and
showed enthusiasm. His discourse is well- paced, productive, not rushed, and unscripted.
