Subject: Request for an update on the follow-up audit/re-audit for ISO 9001:2015
Certification as well as the issuance of PS License.

Dear Sir/ Madam,

With the above-cited subject, the company hereby submits a Letter of Request for an update on
the follow-up audit/re-audit for ISO 9001:2015 Certification as well as the issuance of PS
License since the company believes that we have already complied to the different product
quality testing requirements and other requirements related to Quality Management Systems
which were conducted last November 3-4 2022 and was approved by the DTI-Regional since the
previous year.

Furthermore, we are writing this letter to inform you that we need you to please take actions on
this matter as soon as possible because we are afraid that the company might experience an
increase of information uncertainty as much as decrease of market efficiency. As such, the
reputation and identity of the business from various customers and business partners will also be
affected and critical to its operation.

Additionally, the company would like to ask for information on the following for us to at least be
able to prepare ahead of time:

1. Exact date as to when will the re-audit be conducted.

2. Update on the issuance of the PS License.
3. Added requirements for the follow-up audit/re-audit (if there is/are any).
