AFFIDAVIT

I, BRYAN MICHAEL G. SULATAN, male, of legal age, Filipino, a resident of Block

2, Lot 12, Anami Homes, Mactan, Lapu2x City, after having been duly sworn to in accordance
with law do hereby depose and say THAT:

I am one of the duly authorized representative of HIGEL CONSTRUCTION, a

construction company based in Bayawan City, Negros Oriental;

On 19 September 2023, I conducted a site inspection in the Bohol Police Provincial

Office in Tagbilaran City for the Repair of the Bohol Police Provincial Office, during which
photographs of the site were taken and a Certificate of Site Inspection was issued;

I am executing this Affidavit to attest the veracity of the aforementioned facts and to
support in the submission of the bidding documents with the PRO7, PNP.

IN WITNESS WHEREOF, I have hereunto affixed my signature this 19th day of
