Visa for Cohabitation (visa C)

Please arrange your documents according to this checklist


The file must be lodged in 2 complete sets of photocopies
Only the original certificates will be returned to you
All originals must have an Apostile and notarized translation with an Apostile

A. General requirements

□ Visa application form duly completed on visa-on-web then printed 2 times, dated and
signed

□ Original of passport + copy of data page + copy of all pages with visa and stamps:
 valid for at least three months after the intended date of departure from the Schengen area
 issued within the previous ten years
 containing at least two blank pages

□ Copy of old passport (if applicable, previous Schengen visa)

□ Copy of document proving legal residence in Russia or in the jurisdiction of the


Embassy:
 Russian citizens: copy of internal passport
 Third-country Nationals: residence permit, long-term visa, FMS, valid at least 3 months after
the intended date of departure from the Schengen area

□ Two recent passport sized colour photographs to be glued on the application forms

□ Non-refundable visa application fee to be paid upon submission of visa application

B. Supportive documents

□ Copy of Medical Travel Insurance Policy


 minimum coverage of 30 000 €
 valid in all Schengen countries and covering the first 3 months of your stay

 If travelling with children


 Copy of birth certificate
 Consent of the parental authority from the other parent in the Federation of Russia

C. Specific documents

 From the future partner in Belgium

□ Identification document: copy of the Belgian identity card or resident card (must be minimum
21 years old)

□ Proof of accommodation: property/ownership deed or registered lease

□ Proof of Health Insurance in Belgium (“mutualiteit / mutuelle”)

□ Proof of financial means:


 Declaration of sponsorship (“bijlage 3Bis / annexe 3Bis”) from the Municipality in Belgium
(original + 2 copies)
 Salary slips of the last 3 months, bank statements of the last 3 months and last income tax
assessment paper from the Finance Department in Belgium
 Certificate of family composition issued by the Municipality in Belgium
 From the visa applicant

□ Birth certificate (original + 2 copies), dully legalized and translated (must be minimum 21
years old)

□ Proof of unmarried civil status:


 Non-marriage record certificate (original + 2 copies) not older than 6 months, dully
legalized and translated; OR
 Divorce certificate (original + 2 copies), dully legalized and translated; OR
 Death certificate of the previous spouse (original + 2 copies), dully legalized and translated

□ Non-criminal prosecution certificate (original + 2 copies) not older than 6 months, dully
legalized and translated

□ Medical certificate (original + 2 copies) attesting that you are not carrying any of the diseases
which might endanger public health, from one of the Embassy’s accredited medical centre, not
older than 6 months

□ Proof of relationship:
 Proof that you have cohabited in Belgium or in another country in a legal and uninterrupted
manner for at least one year before the visa application; OR
 Proof that you have known each other for at least two years before the visa application, and
that you have maintained regular contact by telephone or e-mail, and that you have met at
least three times during the two years preceding the request, and that these meetings have
a total of 45 days or more; OR
 Proof that you have a common child (if applicable) and that you are in a permanent and stable
relationship
