1 De casus
Mevrouw Jansen is een vrouw van 80 jaar oud die thuis van de trap is gevallen. Hierbij heeft ze een
collumfractuur in haar linker heup opgelopen. Hierdoor komt ze in aanmerking voor een heuppro-
these. Mevrouw Jansen is altijd gezond geweest en had nog nooit een fractuur of andere ziekte gehad.
Welke prothese past het best bij mevrouw Jansens haar situatie; de totale heuparthroplastiek of de
hemiartroplastiek?
2.2 Discussie
Bij de afweging tussen THP en HP wordt er vooral gekeken naar de levensstijl van desbetreffende
patiënt. Hierbij moet er per patiënt worden gekeken wat de patiënt wil bereiken met de prothese. Om
een uiteindelijke keuze te maken moet er dus per patiënt gekeken worden welke voor- en nadelen van de
prothese een belangrijke rol spelen. De HP kan worden gebruikt wanneer iemand een collumfractuur
heeft. Deze prothese is erg stabiel, heeft weinig kans op luxaties en heeft een grotere range of motion.
Omdat je bij de HP niet de kom vervangt, is het kraakbeen belangrijk bij het soepel bewegen van

het gewricht. Als het kraakbeen dus versleten is zal dit zorgen voor meer pijn, maar ook voor hogere
krachten op de prothese. Een HP is dus vooral een goede optie bij patiënten die nog geen versleten
acetabulum hebben.
De THP kan worden gebruikt wanneer iemand een collumfractuur heeft. Bij de prothese wordt
ook de kom vervangen, waardoor het een grotere ingreep is die langere revalidatie vraagt. De prothese
zorgt voor een betere functie en minder pijn. Wel vindt er in de loop der jaren slijtage plaats van
de polyethyleen liner in het gewricht. Deze slijtage wordt onder andere beı̈nvloed door de grote van
de wrijving. Dit heeft als gevolg dat er osteolyse kan ontstaan als reactie van het lichaam op kleine
polyethyleen deeltjes.
2.3 Patiëntenperspectief
Het is voor mevrouw Jansen belangrijk dat ze zo min mogelijk pijn heeft, zelfstandig kan blijven wonen
en de dagelijkse bezigheden kan blijven doen.
