Vicente, Sto. Thomas, Davao del Norte.

HEALTH THREAT: Poor Environmental Condition

a. Presence of breeding or resting sites of vectors of diseases

CRITERIA SCOR WEIGHT RESULT JUSTIFICATION
E
NATURE OF THE PROBLEM 1 0.66 This is considered a health threat as it
could potentially invite a breeding
SCALE site for vectors of diseases such as,
Wellness State 3 but not limited to, malaria and
Health Deficit 3 dengue; such a likelihood puts the
Health Threat 2 people around the household at risk.
Foreseeable Crisis 1
(González et al. 2020, a survey of tire-
breeding mosquitoes in the
Dominican Republic highlighted the
significance of this issue (González et
al., 2020, p. 507).
MODIFIABILITY OF THE 2 2 The problem is easily modifiable since
PROBLEM it can be transmuted with the
teachings of health education about
SCALE the risks of not maintaining good
Easily Modifiable 3 environmental sanitation.
Partially Modifiable 2
Not Modifiable 1 (Knowledge, Attitude and Practices
Towards Dengue Fever Among Slum
Dwellers: A Case Study in Dhaka City,
Bangladesh, Rahman et al.,2023)
PREVENTIVE POTENTIAL 1 1 There is a high chance of preventing
the presence of breeding sites for
SCALE disease vectors by addressing
High 3 stagnant water, garbage, and other
Moderate 2 conducive environments, they can
Low 1 significantly lower the population of
disease-carrying vectors like
mosquitoes and thereby decrease the
spread of diseases they transmit,
such as malaria or dengue.

(Podder et al.,2019)
SALIENCE 0.5 0.5 The family recognizes it as an ongoing
SALIENCE issue but doesn't view it as needing
SCALE immediate attention or action.
 A Serious Problem, 2
Immediate Action Needed
 A Problem But Not 1
Needing Immediate Attention
 Not A Felt Need/Problem

Total Score Result: 4.16

HEALTH THREAT: Poor Environmental Condition

b. Poor lighting and ventilation

CRITERIA SCOR WEIGHT RESULT JUSTIFICATION
E
NATURE OF THE PROBLEM 1 0.66 The problem is noted to be a health
threat as quality air and adequate
SCALE light help to prevent certain diseases,
Wellness State 3 such as tuberculosis, and also help to
Health Deficit 3 eliminate carbon dioxide and bad
Health Threat 2 odors. Thus, poor lighting and
Foreseeable Crisis 1 ventilation would otherwise have an
adverse result.

(Aditama et al.,2019)
MODIFIABILITY OF THE 2 1.33 The problem is only partially
PROBLEM modifiable, as we, as student nurses,
cannot do anything to change the
SCALE overall structure of the house.
Easily Modifiable 3 Nonetheless, we can still innovate
Partially Modifiable 2 ways to improve air circulation and
Not Modifiable 1 lighting conditions.

PREVENTIVE POTENTIAL 1 1 The magnitude of future problems

that may arise can be highly
SCALE prevented as we find ways to assert
High 3 and address the problem. This
Moderate 2 includes opening the windows as well
Low 1 as clearing anything that obstructs
quality air and lighting from entering
the house.
SALIENCE 1 0.5 During our previous visit, the family
stated that this was a problem, as
SCALE verbalized, "Diri mi gapundo sa gawas
 A Serious Problem, 2 kay hasta ka init ug alimuot sa sulod."
Immediate Action This can also be supported by our
Needed 1 observation that, indeed, no one was
 A Problem but Not inside the house during the few
Needing Immediate Attention
minutes we were there. In any case,
 Not A Felt
0 there is no need for immediate
Need/Problem
action.
Total Score Result: 3.4
HEALTH THREAT: Accident Hazards

a. Fall Hazard
CRITERIA SCOR WEIGHT RESULT JUSTIFICATION
E
NATURE OF THE PROBLEM 1 0.66 Fall hazards pose a health risk due to
the possibility of injuries, fractures,
SCALE and fatalities, necessitating
Wellness State 3 preventive measures to ensure
Health Deficit 3 physical well-being and overall safety.
Health Threat 2
Foreseeable Crisis 1

MODIFIABILITY OF THE 2 1.33 Fall hazards include loosely hanging

PROBLEM roofs, such as tin metal roofs, and
even old trees. While not entirely
SCALE removable, some mitigation to
Easily Modifiable 3 improve safety is possible.
Partially Modifiable 2
Not Modifiable 1
PREVENTIVE POTENTIAL 1 1 This hazard's mitigation includes
securing loose roofs, reinforcing
SCALE structures, and assessing tree
High 3 stability. Regular maintenance and
Moderate 2 timely repairs are critical to reducing
Low 1 the risk of falls and injuries.

SALIENCE 1 0.5 The family acknowledges the issue's

SCALE importance but doesn't view it as
 A Serious Problem, 2 requiring urgent action, potentially
Immediate Action underestimating the risks associated
Needed 1 with the fall hazard.
 A Problem but Not
Needing Immediate Attention
 Not A Felt 0
Need/Problem

Total Score Result: 3.49

PROBLEMS IDENTIFIED AND THEIR LEVELS OF ASSESSMENT AS

WELL AS THEIR DIAGNOSIS

PRESENCE OF HEALTH TREAT

a. 1st Level of Assessment
Poor environmental condition specifically:
Presence of breeding or resting sites of vectors of disease.

Subjective Data:
Verbalized by nanay Rosario “—Nagadaob mi diria usahay,
ginapasuhan namo ang palibot sa balay kay hasta kadaghan ug
lamok.”

Objective Data:
During our recent and past visits, we've noted potential mosquito
breeding sites across their premises. These sites comprise stagnant
water in containers, gutters, flower pots, and improperly discarded
items.

2nd Level of Assessment

Inability to provide a home environment conducive to
health maintenance and personal development due to:
Lack of/inadequate knowledge of importance of hygiene and
sanitation.

b. 1st Level of Assessment

Poor environmental condition specifically:
Poor Lighting and ventilation

Subjective Data:
Verbalized by both interviewees on our previous visit last
August 23, 2023, “Diri mi gapundo sa gawas kay hasta ka init ug
alimuot sa sulod," and “Igang kayo ipundo sa sulod.”
This was further explicated on our recent visit when they said, “Di
man gud mi gaabri ug bintana ba kay basin manulod ang lamok”

Objective Data:
In plain view from the house's front, inadequate lighting and
poor ventilation is evident due to closely spaced structures, numerous
obstructing trees, and closed windows consistently observed during
both recent and previous visits.

2nd Level of Assessment

Inability to make decisions with respect to taking appropriate
health action due to:
Low salience of the problem/condition

c. 1st Level of Assessment

Accident hazards specifically:
Broken chair/ fall hazards

Subjective Data:
As verbalized by both interviewees (ate RoseJean and nanay
Rosario) when asked if there was a flooding problem around the
tin/metal roof) ,”
”O kay tung niagi natumba ang mangga sa among likod, hastang
dakoa biya ato,”

Objective Data:
Our recent and last week's observations revealed a tree-surrounded
area with sections featuring loosely hanging tin roofs. In fact, during
today's interview, a tin roof nearly struck a passerby in front of their
house.
2nd Level of Assessment
Inability to provide a home environment conducive to health
maintenance and personal development due to:
Lack of/ inadequate knowledge of preventive measures.

PRESENCE OF HEALTH DEFICIT

d. 1st Level of Assessment
Seasonal Cough

Subjective Data:
When asked about the common illnesses they encountered
during the past six months, nanay Rosario replied “Usahay ubo ug
sip-on, wala-wala, labaw nag mataligsikan o galain ang panahon.”

Objective Data:
It so happened that during the conduct of our interview, nanay
Rosario coughed (dry) for about a good 15 - 25 seconds.

2nd Level of Assessment

Inability to make decisions with respect to taking appropriate
health action due to:
Lack of trust / confidence in the health personnel/ agency
