JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

WE, ERLINDA B. SETIAR and DONITA C. PEÑALOSA both of legal age, Filipino citizens, both married
and residents of Brgy. Linao, Ormoc City, after having been sworn in accordance with the law hereby depose
and state that:

1. That we are not related either by consanguinity or affinity to GENEVIEVE S. MEDROZO, of legal
age who was married to Jesua C. Orge on June 03, 2001 in Ormoc City;
2. That we personally know as a fact that some documents relating to her particularly on her Marriage
Contract issued by the Local Civil Registrar’s Office of Ormoc City as well as her PhilHealth record,
her surname was erroneously written as MEDROSO instead of MEDROZO as indicated in her
Certificate of Live Birth issued by the Philippine Statistics Authority;
3. That we personally know as a fact that the names GENEVIEVE S. MEDROSO as indicated in her
Marriage Contract and PhilHealth record and GENEVIEVE MEDROZO as indicated in her
Certificate of Live Birth refer to one and the same person;
4. That we are executing this affidavit to attest the truth of the foregoing, and for whatever legal
purpose this affidavit may serve.
