    With reference to your letter No. SO(D)HD/2-15-2022 (322nd) dated March 09, 2022
    regarding the subject cited above.

    With due respect, I would like to bring it to your knowledge that Mr. Muhammad
    Aslam warrantor M/S. Ahmed Enterprises (101, 1 st floor, Manzoor Chamber, Kutchi gali
    No. 1, Marriot Road, Karachi, had a serious medical Problem, he is not medically fit to
    attend the meeting of the caption subject.

    We have attached a medical certificate by a doctor with the description. I request you to
    accept the medical certificate and apologize for the absence from the meeting call by the
    provincial quality control Board, Khyber Pakhtunkhawa.
