9 Description of deaths MDH: 6/1/2022 Experienced Dizziness and loss of appetite. Seen and examined by Resident on duty,
Venoclysis started and labs done on Isolation. -6/2/22 Drooling of saliva

Time of death: 1pm 6/8/2022
Final Diagnosis: Rabies encephalitis.
10 Actions taken MESU: Conducted IEC with family and other within the area. MHO,MESU,BESU, PESU,SI Informed,
LCE informed
Coordinated with ABTC and Department of Agriculture and barangay council. Conducted investigation
for persons who were exposed. Decent burial within 24 hours was observed. Listed person with close
contact and exposed to patient.
16 ESRU Action Conducted IEC with family and other within the area. MHO, MESU, BESU, PESU,
Coordinated with ABTC/department of agriculture and barangay council for animal vaccination.
