CONTRACT OF SERVICE

KNOW ALL MEN BY THESE PRESENTS:

This contract of service made and entered by and between:
The LOCAL GOVERNMENT UNIT (LGU) OF LIGAO CITY, with office
address at New City Hall Complex, Maharlika Highway, Binatagan, Ligao City, Albay
Philippines herein represented by HON. FERNANDO V. GONZALEZ, in his capacity as
CITY MAYOR, herein referred to as the “FIRST PARTY”,
- and -
The DEPED SCHOOLS DIVISION OFFICE OF LIGAO CITY, with office address
at Binatagan, Ligao City, Albay, herein represented by NYMPHA D. GUEMO, in her capacity
as Schools Division Superintendent, herein referred to as the “SECOND PARTY”.

-WITNESSETH-
1. That the first party, in the exigency of service, engages the service of the Second Party
who shall perform work that cannot be performed by the present number of the regular
personnel of the First Party;
2. That the Second Party has signified his/her intention to which the First Party has
accepted, to provide the service needed by the latter;
3. That the Second Party hereby propose highly qualified individuals that possesses the
necessary education, experience and skills required to perform the job as described herein;
4. That said qualified individuals hereby attest that he/she is not related within the fourth
(4th) civil degree of consanguinity and or affinity to the hiring authority and/or the
representative of the First Party; that he/she has not been previously dismissed from
government service by reason of an administrative offense and that he/she has not yet
reached the compulsory retirement age of sixty-five (65);
5. That pursuant to subpar. 4.1.1.1 of par. 4.0 Allowable Expenses Chargeable Against
the SEF under DepED Order no. 10, s. 2017 otherwise known as Revised Guidelines on
the Use of the Special Education Fund to wit:
“Payment of compensation/allowances of teachers locally- hired in elementary and secondary schools
identified to have shortages per the teacher deployment analysis of DepED; the rates of compensation/allowances shall
be determined by the LSB based on funds available; but not to exceed the salary schedule being implemented by the
local government unit (LGU) concerned: Provided that for the purpose of hiring teachers chargeable against the SEF ,
the LSB in each province, city or municipality shall utilize the list found in the Registry of Qualified Applicants
(RQA).”

6. That in view hereof, MS. CRISTINA D. GUEVARRA, of legal age, single/married,

Filipino with postal and residence address at Batang, Ligao City, Albay, Philippines, is hereby
contracted for the period of ten (10) months starting May 2023 to
December 2023, in consideration of the monthly rate of FOUR THOUSAND PESOS

(4,000.00) to be paid every end of the month; That the Second Party is expected to perform the
following functions:
a. Coordinate with community leaders to identify potential learners and learning groups for Basic
Literacy and Accreditation & Equivalency Programs;

b. Advocate ALS Programs and network with other government agencies, non- government
organizations and other people’s organizations for potential support and/or partnership;

c. Establish functional networking and reporting system.

CONDUCT OF LEARNING SESSIONS:

a. Organize a learning group composed of at least fifty (50) learners per ALS Literacy
Volunteer;

b. Determine learning needs of learners;

c. Conduct evaluation to determine the entry and exit level of each learner;

d. Conduct learning sessions using ALS Learning modules supplementary materials for at least
ten (10) months;

e. Device plan and implement them in order to sustain learners in the ALS Program;

f. Conduct home visits, individual tutorials, counselling and other need- driven activities in an
effort to retain and win the learners back to the literacy sessions.
PROGRESS REPORTING
a. The ALS Program Volunteer shall be under the direct supervision of a Public Schools District
Supervisor (PSDS) assigned in the district where he/she assigned;

b. The ALS Program Volunteer is required to submit a monthly report to the District Supervisor
copy furnished the Division Office and Regional Office;

c. Teams will be met on a monthly basis as learning action cells for updates/ problem- solving
to be coordinated by Education Program Specialist in ALS (EPSA) as designated by the
Schools Division Superintendent (SDS);

d. That the Second Party shall perform work at a time and schedule to be agreed upon by both
parties;

e. That it is understood that this contract does not create an employer- employee relationship
between the First Party and the Second Party, that the service rendered hereunder are not
considered and will not accredited as government service and the latter is not entitled to
benefits enjoyed by the regular personnel of the First Party; and,

f. That this contract shall cease to be in force and effect in the event of unsatisfactory
performance of work by the Second Party or for the other causes authorized by law and upon
written notification to be submitted fifteen (15) days prior to the termination of this contract.
