INTERNSHIP/PRACTICUM CONTRACT

PRACTICUM P-2 PROGRAM

I. PURPOSE OF THE CONTRACT/AGREEMENT

a. This AGREEMENT made and entered into by and between:

b. The goal of the internship/practicum program is the provision of practical experience


for the students and observe the practical application of theories learned in the
school.

c. The agreement deals with regulations for an internship/practicum. This


internship/practicum is for a specific period of time and is done as part of a university
degree program.
II. PERIOD

The internship/practicum program runs from March 13 to April 22, 2023.

III. DETAILED DESCRIPTION OF ROLES AND RESPONSIBILITIES

PRACTICUM TRAINEE’S ROLES AND RESPONSIBILITIES:


a. Enter into an internship/practicum contract and/or agreement with the participating
HTE;

b. Sign all the required documents necessary for his or her participation in the
internship/practicum program, including the acceptance letter, internship/practicum
contract and/or agreement;

c. Comply with the provisions of the contract and/or agreement including the rules and
regulations of the college/academic unit, HTE, and CHED at all times;

d. Undergo the required orientation for internship program conducted by the


college/academic unit and HTE;

e. Report for internship/practicum in the HTE based on the schedule indicated in the
internship/practicum contract and/or agreement;

f. Perform tasks and activities indicated in the internship/practicum plan;

g. Maintain confidentiality, when and where appropriate, during and after the
internship/practicum period of all the data, business or trade secrets where such
information is not within the public domain and is indicated or understood to be
confidential;

h. Adhere to the existing rules and regulations of the HTE including the proper use of
tools, instruments, machines, and equipment;

i. Submit a journal of internship as required by HTE reflecting on the approved


internship plan, his or her experiences describing the internship/practicum activities,
any problem/s encountered, and his or her reflections on the internship experience to
the practicum coordinator;
j. Complete the agreed duration of his or her internship/practicum; In case the student
intern/practicum trainee will be unable to finish his or her internship within the
designated period, he or she shall inform the practicum coordinator in writing of his
or her intent and reasons to prematurely end his internship/practicum, at least three
(3) working days before his or her last day of internship/practicum. Failure to
complete the internship/practicum program without valid cause disqualifies the
student intern from retaking the program with the concerned HTE; and

k. Report to the faculty-in-charge for an exit assessment after the completion of the
internship/practicum period.

HTE SUPERVISOR’S ROLES AND RESPONSIBILITIES:


a. Provide the student/trainee with opportunities to work with the specific department or
area where they can best apply the theories, principles and ideas learned in the
academe.

b. Coordinate with the different departments of the partner institution/company/office


where the student/trainee will report.

c. Supervise the student/trainee for the duration of the on-the-job training program.

d. Facilitate the evaluation of the student/trainee’s performance using the agreed upon
standard or requirement of the school/HEI.

e. Regularly meet with the Faculty Practicum Coordinator to discuss his/her evaluation,
observations and other issues that may arise for the duration of the on-the-job
training program. This will also be an opportunity to discuss the program of the
student/trainee.

f. Prepare the evaluation of the student/trainee, upon completion of the on-the-job


training program and submit such evaluation to the Faculty Practicum Coordinator.

PRACTICUM COORDINATOR’S ROLES AND RESPONSIBILITIES:


a. Provide pre-internship/practicum orientation prior to deployment to internship venues
including among others work ethics and anti-sexual harassment laws as pre-
requirement;

b. Coordinate with the Dean or Head of the Unit for the purpose of the
internship/practicum orientation;
c. Inspect internship/practicum venues and sites;
d. Monitor and assess student interns/practicum trainees periodically;

e. Coach or mentor student interns/practicum trainees;

f. Consult and assist student interns/practicum trainees in resolving problems/issues


encountered; and

g. Validate the result of the internship/practicum of students per batch, at the end of the
internship period.

PARENT’S/LEGAL GUARDIAN’S ROLES AND RESPONSIBILITIES:


a. Issues and signs the parent’s written consent; and

b. Co-signs the internship contract and/or agreement to manifest approval to the


internship of their child.

IV. PLACE OF ASSIGNMENT

V. INTERNSHIP PLAN

Internship Plan Goals:

a. Identify key positions in the industry and relate operations to their corresponding
tasks and responsibilities;

b. Define the standard operational procedure in each of the department assigned;

c. Identify and operate the equipment, facilities and lay out design in each of the
department assigned;

d. Perform required skills and techniques necessary in the execution of the operational
procedures of the assigned tasks in accordance with the existing workplace, health
and safety standards; and
e. Analyze and evaluate operational procedure in each assigned task and recommend
any appropriate course of action to be taken to improve procedures.

VI. CONFIDENTIALITY
The Agreement, as well as its terms and conditions herein shall be held in strict
confidence and that no portion or part hereof may be reproduced or be revealed to any
personnel or entity other that the signatories herein or their authorized representative.
