JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

That we, REYNALDO P. SUAREZ, of legal age, Filipino, resident of

Poblacion 1, Gerona, Tarlac and JOHN IBANEZ ZACARIAS, of legal age,
Filipino, resident of Purok 3, Abagon, Gerona, Tarlac, after having been duly
sworn to in accordance with law do hereby depose and say:

1. That we personally know MARCELINO L. SUPNIT who was born on

August 3, 1963 to parents Vicente Supnit and Perfecta Liquigan. We
know these facts because we are and neighbors of their family;

2. That his middle name is LIQUIGAN;

3. That in his Certificate of Live Birth issued by the Philippine Statistics

Authority (PSA), appears that his middle name was erroneously entered
as PERFECTA which is the first name of his mother, instead of Liquigan;

4. That this affidavit is executed to attest to the truth of the matter that the
person named as MARCELINO P. SUPNIT as appearing in his Certificate
of Live Birth, and the person named MARCELINO L. SUPNIT refers to
ONE and the SAME PERSON;

5. That this affidavit is executed freely and voluntarily without any fraud,
duress, force and/or intimidation whatsoever but to confirm and affirm
the aforementioned facts as the basis of any legal intent and purpose it
may serve;

IN WITNESS WHEREOF, I have hereunto set my hands this 24 th day of
