AFFIDAVIT OF DISCREPANCY

I, GINA POSTRERO COSEP, Filipino, married, and a resident of 326

Bogor Building, Bali Oasis, Marcos Highway, Santolan, Pasig City, after having
been duly sworn to in accordance with law, do hereby depose and state THAT:

1. I was born on April 06, 1963 to the spouses JIMENA DIVINA and
SERVILLANO POSTRERO.
2. There is a DISCREPANCY in my Birth Certificate, in which my BIRTH
DATE was erroneously entered therein as “JUNE 03, 1964”, when in truth
and in fact my true and correct BIRTH DATE is “APRIL 06, 1963”, as
evidenced by my Unified Multi- Purpose ID and my Driver’s License.
3. For all intents and purposes, my true and correct BIRTH DATE is “APRIL
06, 1963”.
4. I am executing this affidavit to attest to the veracity of the foregoing for
whatever legal purpose it may serve.
